有高血压家族史的健康子代血浆中降钙素相关肽,血管紧张素及内皮素水平 被引量:8

摘要 目的 检测具有不同高血压家族史的健康子女血浆降钙素基因相关肽(CGRP),血管紧张素Ⅱ(AngⅡ)及内皮素(ET)水平,探讨它们在原发性高血压发生中的作用以及在高血压的一级预防中的意义。方法 随机选择具有不同高血压家族史病人的30~40岁健康子女60例作为研究对象,其中父母双方只有一方患有高血压的30例为单亲组,父母双方均患有高血压的30例为双亲组,选择双亲均无高血压的30例为对照组,采受试者肘静脉血,用放射免疫法测定血浆CGRP、AngⅡ和ET的含量。结果 单亲组血浆中CGRP、AngⅡ浓度分别为(28.3±10.4),(52.5±11.1)pg/mL;双亲组血浆中CGRP、AngⅡ浓度分别为(37.7±14.7),(58.4±9.1)pg/mL,均高于对照组[(CGRP19.9±10.5,AngⅡ45.3±9.9)pg/mL,P<0.01],血浆中ET浓度单亲组(44.8±9.1)pg/mL、双亲组(45.5±7.3)pg/mL稍高于对照组(42.0±7.4)pg/mL,但差异无统计学意义。双亲组与单亲组相比较,血浆中CGRP和AngⅡ的浓度较单亲组高,且差异有统计学意义(P<0.01),ET的浓度两组间差异未见有统计学意义(P>0.05)。血浆中CGRP与AngⅡ呈正相关(r=0.703,P<0.01)。结论 CGRP等血管活性物质的失衡可能是高血压发生的原因,对于有高血压家族史的健康子女,发现血浆中血管活性物质升高,可能是高血压的高危对象,应积极采取一级预防措施防止高血压的发生。 Objective To study CGRP, Ang Ⅱ and ET in healthy offsprings from different families history of essential hypertension. Methods Sixty healthy offsprings with or without a family history of essential hypertension were recruited and divided into two groups: (1) 30 from family with either one parent of essential hypertension; (2) 30 from family with both parents of essential hypertension; (3) control group, 30 subjects from family with parents negative history. The levels of plasma CGRP, Ang Ⅱ and ET were determined by radioimmunoassay. Results The plasma levels of CGRP, Ang Ⅱ in one parent group were(28.35±10.4 pg/mL), (52.55±11. 1 pg/mL) and (37. 75±14. 7 pg/mL), (58.45±9.1 pg/mL) in both parents group, which were increased significantly as compared with those in the control group (CGRP 19.95±10.5 pg/mL, Ang Ⅱ 45.35±9.9 pg/mL, P〈0. 01), however, the plasma levels of ET was similiar among them. Moreover, plasma CGRP and Ang Ⅱ , but not ET, were higher in subjects from two parents group than that in one parent group(P〈0. 01). CGRP was positively related with Ang Ⅱ (r=0. 703, P〈0.01 ). Conclusion The imbalance of vaso-active substances may be the pathogenesis of hypertension. The apparently healthy offsprings from the families with a history of essential hypertension have been shown to have high levels of vasoactive substance, suggesting the primary preventive measures beginning from childhood is advocated.
