摘要
本实验将TGF-β溶解后与经化学处理后的牛松质骨载体在负压下进行充分混匀,冻干后植于兔桡骨中上段1.5cm节段性缺损区。分别于4,8,12,16周取材,观察比较其骨愈合进程。结果显示,术后第12周,实验组中缺损区完全骨性修复,其密度接近正常骨,可见骨髓腔结构;对照组16周时仅骨折端及其尺侧骨膜有轻度反应性增生,呈骨不连征象,缺损区明显可见。实验提示,外源性TGF-β可以启动骨生成过程,对骨折以及骨缺损的修复有促进作用。
Transforminggrowthfactorβ(TGF-β)isoneofmostimportantgrowthfactorswhichregulateboneformation,throughstimulatingmesenchymalproliferationanddiferentiation.Inthisex-perimentalstudy,theefectofTGF-βinstimulatingbonehealinginvivowasconfirmed.Arabbitradialnon-unionmodelwasusedtoevaluatetheefectofTGF-β,whichwasextractedandpurifiedfrombovinebloodplatelets,onthehealingofalargesegmentalosteoperiostealdefect.A1.5centimeterseg-mentaldefectwascreatedinthemid-upperpartoftheradialshaftofadultrabbits.ThedefectwasfiledwithimplantcontainingTGF-βthatconsistedofacarierof60mgofchemicalytreatedbovinecancel-lousbone,reconstitutedwith120μgpurifiedbovineTGF-β.Limbsservedascontrolsreceivedcarieralone.Thedefectswereexaminedradiographicalyandhistologicalyat4,8,12,16weeksafterim-plantation.TheresultsshowedthatinTGF-βimplantgroup,thedefectareaswerefiled,at4weeks,withflocculentradiodensityshadowandflecksofcalcificationandbridgedwithnon-uniformradiodensi-ty.Thecutendsofcortexremainedvisible.At8weeks,thedefectswerebridgedonmedialandlateralsideswithmaterialofuniformradiodensity.Corticesofthecutendswereobscuredbynewbone.Through12to16weeks,thedefectswerebridgedbyuniformnewboneandthecutendsofcortexcouldnotbeseen;whileincontrolgroup,thedefectsremainedclear.Onlyasmalamountofnewboneformedasacaponthecutboneends.TheappearanceoftheTGF-βimplantedsitesinanimalskiledat4weeksshowedhistologicalyalargeareaofchondrocyteslinkedwithcord-shapenewboneandmes-enchymalproliferationanddiferentiationintochondroblast.At16weeks,newlamelarandwovenboneformedincontinuitywiththecutendsofthecortex.Anearlymedularcanalbegantoformandcontainednormal-appearingmarrowelements;incontrast,thedefectsitesofthecontrols,displayedsomesmalareasofwovenboneatthecutendsofthecortex,withfibroustissueonlyinthedefectsite.Remnantsofthecancelousbonecarrierwereobservedinthecontrolspecimen.Thesedatademonstrat-edthatextragenousTGF-βinitiatedosteogenesisandstimulatedthebonedefectsrepairinanimalmod-el.
