糖尿病视网膜血管病变彩色多普勒超声检测与血浆同型半胱氨酸的关系探讨

摘要 目的观察糖尿病视网膜病变时,彩色多普勒超声检测眼动脉(OA),视网膜中央动脉(CRA)和睫状后动脉(PCA)等血流参数,探讨同型半胱氨酸(Hcy)血症与视网膜病变之间的关系。方法同步检测30例正常人,55例糖尿病视网膜病及45例无视网膜病变者的OA、CRA和PCA的彩色多普勒超声以及血浆Hcy水平。结果视网膜病变组中CRA的收缩期峰值流速PVS、舒张期末流速(EDV)、阻力指数(RI)以及PCA中的EDV和RI与正常对照组相比,有明显差异;视网膜病变组中的血糖、Hcy明显升高。结论彩色多普勒超声检查对评估视网膜病变时CRA和PCA的血流动力学变化是一种有效的方法,高Hcy和视网膜病变的发生密切相关,是一种高危因素。 Objective To investigate the relationship between hemodynamies of ophthalmic artery (OA), posterior ciliary artery (PCA) and central retinal artery (CRA) on color Doppler ultrasonography and plasma level of homoeysteine (Hcy) in patients of diabetes. Methods The hemodynamies of OA,PCA and CRA were measured in control group (30 cases), diabetic retinopathy group(55 cases) and diabetic nonretinopathy group(45 cases) using color Doppler uhrasonography. The plasma level of Hcy was measured. Results The value of peak systolic velocity, end diastolic velocity and resistance index in CRA were significantly different from those in controls. The level of serum glucose and Hey increased significantly. Conclusions Color Doppler uhrasonography is an effective method in evaluations hemodynamies of CRA and PCA in diabetic retinopathy. Hyperhomoeysteinemia is a risk factor for diabetic retinopathy.
