输尿管小细胞癌一例

摘要 患者女，62岁。左腰部疼痛伴肉眼血尿1个月余入院。体检：一般状况良好，心肺（-），腹平软，未触及包块，肝脾无肿大。B超见左肾积水，左输尿管中段占位性病变，上端扩张。胸片：肺纹理正常，未见肿块影。CT：左肾盂扩张，左输尿管扩张，输尿管中段软组织密度影。膀胱镜检查未见异常。于2006年2月10日在连续硬脊膜外麻醉下行左肾、输尿管切除术。术中见输尿管肿块2cm×2cm，灰白鱼肉状、质地硬、侵及周围组织。术后病理：左输尿管小细胞（神经内分泌）癌，侵及肾脂肪囊及输尿管外膜。免疫组化：CK（＋）、CgA（＋）、Syn（＋）、NSE（＋）。患者术后刀口愈合后行M-VAC方案化疗1次。
