急性心肌梗死和不稳定性心绞痛患者心率振荡现象的临床研究 被引量:1

摘要 目的 对急性心肌梗死患者和不稳定性心绞痛患者的窦性心率振荡现象进行临床对比观察.方法 将我院2005年1月~2006年1月期间住院的急性心肌梗死患者、不稳定性心绞痛患者及健康对照者各40例,相应分为A、B、C组;利用24h动态心电分析系统获得其室性早搏,分别计算3个组的振荡初始(TO)和振荡斜率(TS)2个指标.结果 A、B、C组的TO均值分别为(0.38±2.84)%、(-0.21±1.97)%、(-0.41±2.36)%,A、C组比较差异有统计学意义(P<0.01);B、C组比较差异有统计学意义(P<0.05);但A、B组比较差异无统计学意义(P>0.05).A、B、C组的TS均值分别为(3.1±2.7)、(8.9±6.4)、(15.2±9.3)mm/R-R间期,3组间两两比较差异均有统计学意义(P<0.01).结论 利用24h动态心电图对振荡初始和振荡斜率2个指标进行测量,可以反映冠心病急性心肌梗死患者和不稳定性心绞痛患者的心率振荡现象,是一种评价其心脏自主神经功能及判断预后很有价值的方法. Objective To contrast the heart rate turbulence phenomenon between the patients with acute myocardial infarction and the patients with unstable angina pectoris. Methods One hundred and twenty patients in our hospital from Jan. 2005 to Jan. 2006, were divided into three groups : acute myocardial infarction group(group A,40 patients) ,unstable angina pectoris group(group B,40 patients),healthy control group(group C,40 patients). To capture ventricular premature beat with 24h dynamic electrocardiography,then to calculate turbulence onset(TO)and turbulence slope(TS)of the three groups respectively. Results TO of group A, B, C was (0.38±2.84) %, (- 0.21 ± 1.97 ) %, ( - 0. 41 ± 2.36 ) % respectively. There were extremely significant difference between group A and C( P〈0. 01 ), significant difference between group B and C(P〈0.05), no statistical difference between group A and B(P〉0.05). TS of group A,B,C was (3. 1±2.7) ,(8.9±6.4) ,( 15.2±9.3)mm/R-R respectively, there were extremely significant difference between every two among three groups(P〈0.01). Conclusion To measure parameters of TO and TS with 24h dynamic electrocardiography, is a preferable method for evaluating autonomic nervous system function of heart, which can reflect the phenomenon of heart rate turbulence in patients with acute myocardial infarction and unstable angina pectoris.
