地塞米松抑制试验在亚临床库欣综合征诊断中的作用 被引量：2

摘要 目的评价地塞米松抑制试验对肾上腺意外瘤中亚临床库欣综合征诊断的价值。方法收集67例肾上腺意外瘤患者的临床资料,根据小剂量(2 mg)及大剂量(8 mg)地塞米松抑制试验(DST)结果分组。组1(n=11):2 mg及8 mg DST均不能抑制;组2(n=15):2 mg DST不能抑制,8 mg DST能抑制;组3(n=41),2 mg DST能抑制。对三组患者的临床症状、生化指标及激素检测值进行统计分析。结果与组3比较,组1和组2患者的肥胖、高血压、糖尿病及高血脂的患病率以及腰臀比、收缩压、餐后2 h血糖、甘油三酯、总胆固醇、低密度脂蛋白、8:00和24:00血皮质醇、24 h尿游离皮质醇均增高,促肾上腺皮质激素(ACTH)降低;上述指标在组1和组2间无明显差异。组1所有患者除DST不能抑制外,还伴有其他至少1项下丘脑—垂体—肾上腺轴(HPA)功能紊乱症状。结论小剂量DST对肾上腺意外瘤中亚临床库欣综合征的诊断有较大价值。
