关节镜下微骨折技术治疗膝关节软骨缺损的近期临床疗效评价 被引量:11

摘要 目的评价关节镜下微骨折技术治疗膝关节全层软骨缺损的近期临床疗效。方法对2002年1月~2005年1月的48例Outerbridge分型Ⅲ~Ⅳ级股骨髁及滑车区域的全层软骨缺损患者.在膝关节镜下行软骨缺损区微骨折术.术后平均随访12.3个月.根据改良Cincinnati膝关节评分系统、国际膝关节论证委员会(International Knee Documentation Committee,IKDC)评分和Lysholm评分综合评价临床治疗效果。结果34例(70.8%)临床症状改善。12例(25%)症状改善不明显.2例(4.2%)效果欠佳;3种国际公认的评分系统评分结果术后较术前均明显增加。差异有统计学意义(P〈0.01)。结论采用关节镜微骨折技术修复膝关节软骨损伤可以明显改善术后功能。缓解症状.在没有更好的替代技术出现前.关节镜下微骨折技术仍然具有优势和潜能。 Objective To evaluate the short-term clinical effect of arthroscopic microfracture technique for the treatment of full thickness articular cartilage lesions in knee. Methods From Jan. 2002 to Jan. 2005, 48 patients were treated using the arthroscopic microfracture technique. Modified 10-point scales of the Cincinnati knee rating system ,IKDC and Lyshom scoring system were used to measure outcomes at mean follow-up of 12.3 months. Results At the time of the latest follow-up, knee function was rated good to excellent for 34 patients (70.8 % ), fair for 12 patients (25%), and poor for 2 patients (4. 2 % ). Three scoring system improved obviously (P〈0.01). Conclusion Arthroscopic microfracture technique has advantages and potential for the treatment of full thickness articular cartilage lesions in the knee than other ways.
