摘要
目的:探讨角膜溃疡穿孔的不同临床治疗方法及其疗效评价。方法:收集我院1993-01/2003-06角膜溃疡穿孔病例49例(49眼),进行回顾性分析,对不同病变所采用的不同治疗方法进行比较、评价各种治疗方法的疗效。结果:49例患者中有16例经单纯药物治疗治愈,其中6例患者视力获得不同程度的提高;33例单纯药物治疗效果不佳患者经手术治疗治愈,其中22例穿透性角膜移植患者中有17例患者视力不同程度提高。单纯药物治疗遗留粘连性角膜白斑13例,单纯性角膜白斑3例,结膜瓣掩盖术后遗留粘连性角膜白斑3例。术后继发青光眼2例。角膜移植患者术后有6例发生角膜移植排斥反应。无溃疡复发。结论:药物治疗是有效的方法之一;穿透性角膜移植术是最佳的手术治疗方法。
AIM： To study the different kinds of clinical treatments on cornea ulcer with perforation, and to evaluate their prognosis.
METHODS： A total of forty-nine cases of cornea ulcer perforation from January 1993 to June 2003 were collected and retrospective analysis was performed. Different ways of cliniCal treatment were compared and the prognosis was evaluated.
RESULTS： In 16 of 49 cases, the ulcers were healed after simple drug therapy. In 33 cases, the ulcers were healed after the combined therapy of drug and surgery. For 6 of 16 cases with simple drug therapy, the vision acuity was increased, and for other 10 cases, the vision acuity was not significantly improved or decreased little. For 17 of 22 cases of penetrative cornea transplantation, the vision acuity was increased, and for other 5 cases, no difference was observed. The vision acuity of 2 patients with lamellar cornea transplantation was not changed at all. The vision acuity of 2 patients with conjunctive flap was not changed, neither. Eighteen transplants of 24 cases of keratoplasty were transparent, 5 were semi-transparent, 1 was cloudy. Adherent leukoma of cornea was found in 13 cases, which undertook the simple drug therapy and 3 cases by conjunctive flap. Simple leukoma of cornea were found in 3 cases of simple drug therapy. Glaucoma was developed for 2 cases after operation and glaucoma decompression was performed. Transplant rejection was developed in 6 patients after keratoplasy and the condition was improved after drug therapy. No recurrence of ulcer was found for all the cases after 6-12 months of follow-up.
CONCLUSION： Drug therapy is a effective method, penetrative keratoplasty was the best method available.
