Relationship between plasma lipoprotein-associated phospholipase A_2 activity and coronary heart disease

摘要 目的:探讨血浆脂蛋白相关性磷脂酶A2(Lp-PLA2)活性与冠心病(CHD)传统危险因素的关系及与CHD发病的关系。方法:测定经冠状动脉造影证实的87例CHD患者和58例对照组的血浆Lp-PLA2活性、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平并进行比较;同时调查年龄、性别、高血压史、糖尿病史、高脂血症史及吸烟史等CHD危险因素,研究它们与Lp-PLA2活性的关系。结果:①CHD组血浆Lp-PLA2活性显著高于对照组(P<0.01)。②CHD组中男性并高脂血症者Lp-PLA2活性显著升高。血浆Lp-PLA2活性与TC、LDL-C呈正相关,与HDL-C呈负相关,而与年龄、高血压、糖尿病、吸烟、TG无关。③非条件Logistic多元回归分析显示,调整其他危险因素后,Lp-PLA2活性仍与CHD独立相关。结论:CHD患者血浆Lp-PLA2活性显著增高,与TC、LDL-C呈正相关,与HDL-C呈负相关,是CHD的独立危险因素。 Objictive: To investigate the correlation between plasma lipoprotein-associated phospholipase A2 (Lp-PLA2) activity and some traditional risk factors of coronary heart disease(CHD), and the relationship between Lp-PLA2 activity and the incidence of CHD. Method: Lp-PLA2 activity and the levels of total cholesterol (TC) ,triglyceride(TG), high density lipoprotein-cholestero[ (HDL-C) and low density lipoprotein-cholesterol (LDL-C) were measured in 87 subjects with angiographically confirmed CHD and 58 control subjects. Risk factors of histories including essential hypertension ( EH ) , diabetes mellitus ( DM ) , hyperlipoproteinemia and smoking status were obtained from all participants via a questionnaire. Result: ①Plasma Lp-PLA2 activity was significantly higher in the CHD group than in the control group( P 〈0. 01). ②Males had higher Lp-PLA2 activity than females. CHD patients with hyperlipoproteinemia also had higher Lp-PLA2 activity. Lp-PLA2 activity correlated positively with TC.LDL-C and negatively with HDL-C. There was no correlation between Lp-PLA2 activity and age, TG, history of EH, DM, smoking status. ③Based on multivariate non-conditional logistic regression analysis, in a model adjusted for traditional risk factors,the association of Lp-PLA2 activity with CHD was statistically significant and independent. Conclusion:Lp-PLA2 activity elevated significantly in CHD subjects. Lp-PLA2 activity correlated positively with TC.LDL-C and negatively with HDL-C. Lp-PLA2 activity is an independent risk factor of CHD.
