摘要 为了探讨电视胸腔镜辅助颈部低领式切口切除前上纵隔肿瘤的手术方式和应用价值,自1995年3月~2005年3月,对48例前上纵隔肿瘤行电视胸腔镜辅助颈部低领式切口切除术,其中坠入性甲状腺肿瘤21例(恶性2例,良性19例),胸腺瘤18例,脂肪瘤4例,淋巴囊肿3例,皮样囊肿1例,心包囊肿1例。手术均顺利完成,无手术死亡和严重并发症,手术平均时间(69±11)min,术中平均失血量(85±15)mL,平均术后住院(8±1.5)d。初步研究结果提示,电视胸腔镜辅助颈部低领式切口切除前上纵隔肿瘤,可达到传统经胸手术效果,手术显露充分,操作确切可靠,创伤小、恢复快,有重要临床应用价值。 The objective of this study was to evaluate the clinical significance of surgical treatment for patients with anterior superior mediastinal tumors by using thoracoscopy combined with neck transverse incision. From march 1995 to march 2005 video-assisted neck transverse incision resections were performed for 48 patients with anterior superior mediastinal tumors including 21 substernal soiters (2 of them were malignant) 18 thymomas, 4 lipomas, 3 cystic lymphangiomas, 1 dermoid cyst and 1 cyst of pericardium. Results: Forty-eight patients underwent successful operation and quickly recovered without any severe complication or death. The mean operating time was (69±11)min, the mean operating blood lose was (85±15) mL,the mean duration of postoperative hospital stay was (8±1.5) d. Conclusion: Videoassisted neck transverse incision in the treatment of patients with anterior superior mediastinal tumors can obtain the same resection outcome as the routine open thoracotomy and have many advantages such as satisfactory exposure, precise operation, minimal injury, and quick recovery.
