PCNA与CK34βE12在前列腺癌中表达的临床意义 被引量:4

摘要 目的观察增殖细胞核抗原(proliferatingcellnuclearantigen,PCNA)与高分子量细胞角蛋白(CK34βE12)在各种前列腺良恶性病变的诊断和鉴别诊断中的应用价值。方法应用免疫组织化学LDP法检测68例前列腺癌、235例前列腺良性病变组织中PCNA及CK34βE12的表达情况。结果①PCNA在前列腺癌组织中表达较前列腺增生组织增加,差异有显著性(P<0.001);PCNA的表达随着组织学分级及临床分期增加而增加(P<0.001);PCNA高表达组(3、4级)术后生存率明显低于低表达组(1、2级)(P<0.01)。②前列腺癌基底细胞均丢失,而前列腺良性病变基底细胞绝大多数保存完好,只有少数3级前列腺上皮内肿瘤和非典型性瘤样增生基底细胞不完整。③CK34βE12表达阴性时,PCNA表达越强,肿瘤恶性程度越高,预后越差。结论联合检测PCNA与CK34βE12在前列腺良、恶性疾病的诊断与鉴别诊断及对肿瘤恶性程度和预后的判断中具有很好的应用价值。 Objective To observe the value of proliferating cell nuclear antigen(PCNA) and CK34 βE12 in diagnosis and differentiation of benign and malignant prostate lesions. Methods The expression of PCNA and CK34 βE12 in 68 cases of prostatic carcinoma and 235 cases of benign prostatic tissues were detected by Immunohistochemical LDP method. Results ① The expression of PCNA in carcinoma tissue was significantly higher than that in the BPH tissue(P〈0.001). The expression of PCNA increased with the increases of pathological grade and clinical staging (P〈0.001). There was a significant difference in survival rate between PCNA high expression (grade 3, 4) and low expression (grade 1, 2) cases ( P 〈 0.01). ② It was demonstrated that the basal cell layer was lost in all prostatic carcinoma, but existed in most of the proliferative lesion, except for a few cases of typical adenomatous hyperplasia and grade 3 intraepithelial neoplasia, in which the basal layer was disrupted. ③ When the expression of CK34 βE12 was negative, the expression of PCNA was higher; the tumor differentiation was higher; and the prognosis was poorer. Conclusion The study showed that the combined detection of PCNA and CK34 βE12 were very useful in the diagnosis and differential diagnosis of benign and malignant prostate lesions and for the assessment of malignancy degree and the prognosis of carcinoma.
