摘要
目的探讨高场MR在主动脉病变中的诊断价值,着重分析各种主动脉疾患合理的检查程序和序列的优化选择。方法2004年9月至2005年3月行主动脉MR检查的32例阳性患者,采用西门子1.5T超导MR行屏气或自由呼吸黑血和(或)亮血非对比剂增强MR检查后,行屏气或自由呼吸主动脉对比剂增强三维MR血管成像(3DCEMRA),4例患者随后行屏气四维主动脉血管成像。结果平均扫描时间26min(15~55min),除3例行自由呼吸MR扫描外,其余均在屏气下检查,所有患者均获得了具有诊断价值的MR图像,作出了正确的诊断。黑血及亮血二维序列清楚地显示内膜片、主动脉管壁及周围解剖信息,亮血电影序列显示局部异常血流,提示相应的主动脉(瓣)病变,CEMRA则全程显示主动脉腔及其主要分支的三维或三维动态充盈情况。结论高场MR结合快速扫描序列,可根据具体情况灵活采用合适的检查程序和优化的扫描序列获得准确的诊断。
Objective To evaluate the application value of high field magnetic resonance imaging in diagnosing aortic diseases and select the optimal MR protocol and sequences for different aortic disease and patients. Methods From Sep. 18^th, 2004 to Mar. 31^th, 2005, a total of 32 patients(male 20, female 12, average age 46. 63 y, ranging from 17-75 y) with aortic disease were included in present study. A 1.5 T MR system were used to scan all the patients. Multiple fast sequences including HASTE, TSE, TrueFISP was initially applied before contrast enhancement. Subsequently, Fast low angle shot (FLASH) sequence and post-processed three-dimensional (3D) reconstruction software were used to obtain 3D contrast enhanced MR angiography (3D CE MRA). 4 patients were performed with time-resolved CE MRA. Results The mean duration of scan time was 26 minutes ranging from 15-55 minutes. All patients except 3 were scanned with breath-hold sequences. And all examinations were accomplished successfully with good quality of diagnostic MR imaging. 2D dark and bright blood sequences provided detail information of the intima flap of the aortic dissection, the wall of the aorta and the anatomic abnormality of the adjacent tissues and organs. A True FISP cine sequence demonstrated the abnormal flow state of the disease and could evaluate the aortic valve disease accurately. 3D contrast enhanced MRA with large FOV and parallel acquisition technique clearly and accurately displayed the extension and the severity of the disease of the aorta and its main branches, while 4D CE MRA demonstrated the dynamic course of contrast material filling the aorta and its branches. Conclusion With the advantages of high field MR and multiple fast novel sequences, we should choose optimal MR protocol and different sequences purposively in patients with all kinds of aortic disease to evaluate the lesions accurately and effectively.
