紫杉醇用于兔青光眼手术的实验研究 被引量:2

摘要 目的:探讨紫杉醇在抗青光眼滤过术后抑制成纤维细胞增殖的作用。方法:将64只新西兰大白兔随机分为4组:空白对照组,标准对照组,实验Ⅰ组,实验Ⅱ组,均行右眼标准小梁切除术。空白对照组不用药物,其他3组术中分别在巩膜瓣下一次性应用0.3 mg/ml丝裂霉素C0、.2 mg/ml紫杉醇、0.3 mg/ml紫杉醇5 min。分别在术后第3、7、14、28天时检查眼压、滤过泡、结膜、角膜、眼底情况,取滤过泡及附近组织送病理,并用免疫组织化学(Ki67)方法观察成纤维细胞增殖情况。结果:术后第7、14、28天4组眼压、功能性滤过泡、免疫组化染色阳性细胞核数比较,实验Ⅱ组疗效优于其他3组(P均<0.05);实验Ⅱ组未发现明显副作用,标准对照组可见结膜伤口漏及角膜上皮损伤。结论:青光眼滤过术中应用紫杉醇能抑制纤维增殖,有助于功能性滤过泡的形成,有效降低眼压,且无明显副作用。 Objective: To explore the role of paelitaxel in inhibiting the fibroblast proliferation after the glaucoma filtering surgery in a rabbit model. Methods: Sixty-four New Zealand White rabbits were randomly divided into four groups: control group Ⅰ , control group Ⅱ , experimental group Ⅰ , and experimental group Ⅱ . Each right eye was given standard trabeculectomy. The control group Ⅰ received no treatment during trabeculectomy. The other three groups were given 0.3 mg/ml MMC, 0.2 mg/ml paclitaxel, and 0.3 mg/ml paclitaxel respectively for 5 minutes. The filtering blebs were observed and the filtering site was examined by light microscope, and the number of positive cell nucleus were counted under microscope area after immunohistochemical staining at the 3rd, 7th, 14th, and 28th day after surgery. Resalts: The changes of the intraocular pressure, fotering blebs, and number of Ki67 positive cell nucleus in e:cperimental group Ⅱ were significantly different compared with those in other three groups(all P 〈 0.05 )at the 7th, 14th, and 28th day. Experimental group Ⅱ had a better effect on inhibiting fibroblast proliferation and had fewer side effects. Injured corneal epithelium and conjunctival wound leak were found in the control group Ⅱ . Conclusion: Paclitaxel can inhibit the fibroblast proliferation, decrease the intraocular pressure, and keep the filtering passage unobstructed after glaucoma filtering surgery.
