摘要
目的探讨人体质量指数(BMI)对髋臼骨折术后病人并发症的影响,更好地指导临床护理实践。方法对85例行切开复位内固定手术的髋臼骨折病人进行回顾性分析。根据BMI将病人分成4组:非肥胖组25例(BMI≤24),超重组26例(BMI24.1~28)。肥胖组28例(BMI28.1~30),病理性肥胖组6例(BMI〉30)。根据有关评价标准观察切口感染、神经麻痹、深静脉血栓形成(DVT)和异位骨化等并发症的发生情况。应用卡方检验比较不同组间术后并发症的发生率;并以非肥胖组为时照,计算部分组的比值比(OR)和95%可信区间。结果①各组间术后并发症的发生率差异有显著性统计学意义。②非肥胖组与超重组比较,以度肥胖组与病理性肥胖组比较,术后并发症的发生率差异无显著性统计学意义(P=0.4282,P=0.6445);而肥胖组、病理性肥胖组与非肥胖组比较,术后并发症发生率的差异有显著性统计学意义(P=0.0038,P=0.0421);同时,该两组与超重组比较,术后并发症发生率的差异亦有显著性统计学意义(P=0.0304,P=0.0456)。③与非肥胖组比较,肥胖组DVT的发生率是其2.6倍(OR=2.6,95%可信区间为1.1~5.8,P=0.03);与非肥胖组比较,病理性肥胖组切口感染的发生率是其5倍(OR=5,95%可信区间为1.2~23.2,P=0.05);肥胖并没有增加术后神经麻痹和异位骨化的发生率。结论髋臼骨折术后病人,BMI的增加会对并发症的发生率产生影响,护理人员应给予更多关注,并采取相应预防措施。
Objective To explore the relationship between body mass index (BMI) and postoperative complications of patients with acetabular fractures. Methods A retrospective study was used to analyse the data of 85 patients with acetabular fractures treated with open reduction and internal fixation . The patients were divided into 4 groups according to their BMI. non-obese (BME≤24,n=25), overweight(BMI 24.1-28,n=26) ,obese(BMI 28.1-30,n=28) ,and morbidly obese(BMI〉30,n= 6). The states of wound infection, nerve palsy, thrombosis of deep venous and heterotopic ossification were evaluated. Pearson chisquare tests with exact significance were used to compare the prevalence of postoperative outcomes. Results BMI related to the wound infection and thrombosis of deep venous. Odds ratio analysis revealed that obese subjects were 2.6 times more likely than patients of non obese weight to have thrombosis of deep venous. Morbidly obese patients were 5 times more likely than morbildly obses patients to have wound infection. Conclusions BMI is predictablility of postoperative complications of patients with acetabular fractures,and nurses should give more attention.
