慢性肝炎组织中CD95(Fas/APO-1)抗原的分布及其意义 被引量:1

Distribution of CD95 (Fas/APO-1 ) Antigen in the Tissue of Chronic Hepatitis

摘要 CD95(Fas/APO-1)是存在于细胞表面介导细胞凋亡的膜受体分子,属于TNFR/NGFR超家族的成员。本文用免疫组织化学法研究Fas抗原在乙型和丙型肝炎病毒引起的慢性肝炎肝组织中的分布,探讨细胞凋亡启动因子Fas在病毒性肝炎发病机制中的作用。结果显示,Fas抗原表达阳性细胞在肝小叶中多呈散在或灶状分布于肝小叶周边靠近汇管区的碎屑样坏死区内,周围有淋巴细胞浸润,但也有少数病例Fas抗原表达在肝小叶内呈弥漫分布,一般局限于某个肝小叶。Fas抗原仅表达于肝细胞浆和/或胞膜上且呈细颗粒状均匀分布,核内未见表达,但其表达的肝细胞又仅限于核心抗原阳性和/或HBxAg阳性的肝细胞。随着慢性肝炎组织炎症活动性的增加及纤维化程度的增力。,Fas抗原表达的阳性率也显著增加(P<0.05)。Fas抗原表达阳性的病例在活检时的血清ALT浓度(83.5±27.4IU/ml)明显高于该抗原表达阴性者(26.5±18.7IU/ml)(P<0.001),而与患者的年龄和性别无关。结论:Fas抗原表达的阳性率与慢性肝炎的活动度及肝纤维化的程度相关;Fas杭原表达阳性的病例血清ALT水平高,提示Fas高表达于病变较严重的组织;Fas抗原? in this study,we investigated the distribution of CD95 in the tissue of chronic hepatitis to determine the possible role of CD95 (Fas/APO-1) antigen during the progression of chronichepatitis. Method:Liver biopsy samples from 52 chronic hepatitis, 8 liver aneurysm and 5 normal liver were fixed in formaldehyde and embedded in paraffin. The liver specimens were immunohistostained by ABC method with rabbit anti-Fas ployclonal antibody(Santa Cruz CO. USA) and ABC kit (Vector Lab CO. USA). Histological activity ofchronic hepatitis was quantitatively analyzed by Histologic Grading System sponsored by IXth Biennial Scientific Meeting Asian-Pacific Association for the Study of the Liver. Result:Fas antigen was not detected in normal liver tissues. The specific positive signal was found mainly on the surface or/and in the cytoplasm of the hepatocytes and biliduct epithelium infected by HBV and/or HCV. These positive hepatocytes were scattered nearby periportal region with 'piecemeal necrosis'. The infiltration of mononuclear cells was occasionally observed. The prevalence of Fas antigen expression was closely correlated with the grade of inflammatory acitivity or the stage of fibrosis. Fas antigen positive cases demonstrated higher ALT levels in serum (85. 3± 27. 4 IU/ml verse 26. 5 ± 18. 7 IU/ml. P<0. 001 ). Conclusion: 1. Fas antigen expression was closely correlated with the histological activity of chronic hepatitis. 2. Only HBV or/and HCV infected hepatocytes expressed Fas antigen and this suggest that virus aggression may activated Fas-mediated signal transduction pathway.
