摘要
目的探讨不同大小原发性肝癌(HCC)的血流分布和彩色多普勒血流参数改变。方法对123例肿块型HCC进行了CDFI检测,按肿块直径大小分为3组:A组(≤50mm),B组(50mm<直径≤100mm),C组(>100mm)从HCC的血流分布和血流参数方面进行了研究。结果肝癌肿块内部和周边血流显示率B组大于A组,C组大于B组,3组两两比较差异均有显著性(P<0.01);动脉峰值血流速和阻力指数B组和C组均高于A组(P<0.05),但C组与B组比较差异无显著性(P>0.05);门静脉血流速A组高于C组(P<0.05),但A组与B组,B组与C组比较差异无显著性(P>0.05)。结论HCC的血流分布以及血流参数与肿块的大小有明显关系,通过对其进行研究,有利于了解肝癌肿块的变化发展过程,为HCC的不同阶段对其进行诊断和治疗具有重要意义。
[Objective] To explore the blood flow distribution and blood flow parameter change of hepatoceUuler carcinoma with differential size by color doppler flow imaging (CDFI). [Methods] 123 hepatocelluler carcinoma were examined, they were divided into three groups according to the size of mass, the A group (≤50 mm), B group (50 mm〈diameter≤100 mm) and C group (〉100 mm), the blood flow distribution and blood flow parameter were contrasted and studied between the three groups. [Results] There was a higher blood flow display rate in mass and around mass in the B group than in the A group and in the C group than in the B group, there was statistical difference between three groups in blood flow distribution (P 〈0.01). The peak value of blood flow and the mean resistance index were higher in the B group and C group than in the A group, there was statistical difference between them (P 〈0.05), however, there was not statistical difference between the B group and C group (P 〉0.05), the velocity of the portal vein was higher in the A group than in the C group (P 〈0.05), but there was not statistical difference between the A group and B group and between the B group and C group (P 〉0.05). [Conclusion] There is an obvious relation between the mass size of hepatocelluler carcinoma and their blood flow parameter and blood flow distribution, it is beneficial for us to understand the process of change and development of hepatocelluler carcinoma by studying them, there is an important significance for us to diagnose and cure hepatoceUuler carcinoma in different period.
