摘要 目的:研究脑动静脉畸形与海绵状血管瘤的MRI及MRA特征,评价不同的成像方法对脑血管畸形的诊断价值。材料与方法:对21例脑动静脉畸形及6例海绵状血管瘤分别作了常规MR成像及MR血管成像,MRI采用SET1和TurboSET2加权序列,MRA采用2D-FLASH及3D-FISP序列,6例海绵状血管瘤MRA仅使用2D-FLASH序列。所有成像以环形激化头线圈在1.0TMR仪上完成。结果:T1及T2加权成像畸形血管表现为无信号的流空区,AVM的内部结构及继发改变MRI能很好显示,FISP-3D成像,17例显示供血动脉,11例引流静脉在2D-FLASH成像时显示。脑海绵状血管瘤在T1及T2加权像上表现典型,FISP-3DMRA未发现有病变血管显示。结论:MRA能显示AVM的三维解剖结构,采用不同的成像方法,能完整显示供血动脉及引流静脉与瘤巢的关系,MRA和MRI联合使用有利于对血管结构及周围软组织结构作出全面评价。T1及T2加权成像能对脑海绵状血管瘤作出明确诊断,MRA并不能提供更多的影像诊断信息。 Objective: To study MRI and MRA features of intracranial arteriovenous malformations (AVM) and cavernous hemangioma, and to evaluate the clinical value of different imaging methods in diagnosing intracranial vascular malformations.Materials and Methods: 21 patients with intracranial AVM and 6 patients with cavernous hemangioma were studied respectively with conventional MR imaging and MR angiography. MR imaging was performed with SET 1 and Turbo SET 2 weighted sequences, and MR angiography was done with 2D FLASH and 3D FISP sequences. In 6 patients with cavernous hemangioma, only 2D FLASH sequence was used. All MR studies were accomplished on a 1.0T MR system using a circularly polarized head coil.Results: Typical sign of signal void can be seen on both T 1 and T 2 weighted images of AVM. The internal structure and the secondary changes of AVM were well defined on MRI. The feeding arteries were seen on 17 cases with 3D FISP MRA, while draining veins were shown in 11 cases with FLASH 2D MRA. The cavernous hemangioma revealed the typical characteristic on both T 1 and T 2 weighted images. 3D FISP MRA failed to reveal the pathologic blood vessels of the lesions.Conclusion: MRA may demonstrated three dimensional anatomy of AVM. With different imaging methods, the lesions and its feeding arteries and draining veins can be well defined. MRA when combined with MRI, may aid in the comprehensive evaluation of the vascular structures and its surrounding tissues. A definite diagnosis of cavernous hemangioma can be given on both T 1 and T 2 weighted images, and MRA can not provide further information.
