经阴道超声检测不孕症患者输卵管积液的价值 被引量:3

The diagnosis of hydrosalpinx about infertility with transvaginal sonography

摘要 目的观察输卵管积液在不孕症患者月经周期不同阶段的出现情况,评估经阴道超声诊断输卵管积液的准确性。方法对1704例不孕症患者常规采用经阴道超声进行病因学筛查,分别在月经周期的卵泡期、排卵期、黄体期至少进行3次超声检查,观察输卵管积液出现时间、大小,发现输卵管积液的部分患者进行腹腔镜检查,计算其符合率。结果94例出现输卵管积液患者中,排卵期及黄体期积液的输卵管大小(2.3±1.1)cm、(2.0±1.3)cm及阳性率(87.2%、78.7%)差异均无统计学意义(P>0.05),而卵泡期积液大小(1.2±1.3)cm及阳性率(48.9%)较排卵期及黄体期低(P<0.05);31例患者进行了腹腔镜检查,显示经阴道超声的诊断符合率为96.8%。结论经阴道超声在排卵期及黄体期更易发现输卵管积液,在月经周期的不同阶段进行超声检查有助于更好地发现输卵管积液,经阴道超声检查可作为不孕症患者进行病因学筛查的一种准确、简单手段。 Objective To observe the changes of hydrosalpinx in different phase of sufferers' menstrual cycle and assess the accuracy of transvaginal sonography. Methods One thousand seven hundred and four cases of infertility were checked with transvaginal sonography according to the theory of etiology. Transvaginal sonography were performed at the follicular phase, ovulation phase and luteal phase to detect the timing and sizes of hydrosalpinx at least three times, Laparoscopy was done at part of cases with hydrosalpinx and was compared with transvaginal sonography also, And then, the ratio of accord was calculated. Results In 94 cases of hydrosalpinx, the size and positive ratio of hydrosalpinx were quite similar at ovulation phase and luteal phase [(2.3 ± 1.1)cm vs (2.0± 1.3)cm, 87.2% vs 78.7%, P 〉 0.05], However, the size[( 1.2 ± 1.3)cm] and positive rate (48.9%) of hydrosalpinx at follicular phase were lower than those at ovulation phase and luteal phase( P 〈0.05). In 31 cases, comparing transvaginal sonography with laparoscopay, the accurate ratio was 96.8%. Conclusions Hydrosalpinx was easier to be dectected at ovulation phase and tureal phase by transvagianl sonography. Transvaginal sonography could be used in detecting hydrosalpinx at different phases of menstrual cycle. Transvaginal sonography is a simply and accurately diagnostic tool for infertility.
