腹腔镜二孔法及三孔法阑尾切除术的临床观察 被引量:3

摘要 目的探讨二孔法及三孔法腹腔镜阑尾切除术的临床应用价值和手术适应证的选择。方法回顾性分析腹腔镜二孔法阑尾切除术(二孔法组)99例和三孔法阑尾切除术(三孔法组)70例的临床资料,并与传统开腹阑尾切除术(开腹组)102例进行对比。结果腹腔镜阑尾切除术组与开腹组相比,在术后肛门排气时间、住院时间等方面差异有统计学意义(P<0.05),具有明显优势,但二孔法手术时间与开腹组相当,差异无统计学意义(P>0.05),三孔法手术时间与开腹组相比,差异有统计学意义(P<0.05)。二孔法组与三孔法组相比,二孔法在手术时间上具有明显优势。随访结果显示腹腔镜阑尾切除术远期并发症明显减少。结论腹腔镜二孔法阑尾切除术,操作简单,易于推广;三孔法则具有处理复杂阑尾切除术的优势。腹腔镜阑尾切除术具有创伤少、并发症发生率低、恢复快的优点,但应注意适应证的选择。
