摘要
目的研究骨形态发生蛋白-2(BMP-2)对悬吊固定后肌腱在骨隧道内愈合方式的影响。方法采用兔膝关节半腱肌重建前十字韧带悬吊固定模型,对照组为假手术组而试验组在隧道内植入骨形态发生蛋白-2。分别于手术后4、8、12周采取组织,采用苏木精-伊红染色、天狼猩红染色,观察骨隧道和肌腱移植物间的界面组织学特殊变化,分析腱-骨愈合。结果4周对照组腱-骨间有腱-骨分离,BMP-2组腱-骨间充满结缔组织;8周BMP-2处理组形成Sharpey纤维;12周时BMP-2组形成大量Sharpey纤维,对照组12周时开始出现Sharpey纤维,各组在12周时均无直接连接形成。结论悬吊固定腱-骨愈合发生在隧道深部,BMP-2可以加速腱-骨悬吊固定后腱-骨之间的愈合,腱-骨之间以间接连接方式愈合。
Objective To investigate the effect of the recombinant human bone morphogenetic protein- 2 (BMP - 2) on the healing pattern of a tendon in bone tunnel with the suspending fixation. Methods The anterior cruciate ligament reconstruction with semitendonous tendon was done in knee joints of New Zealand rabbits by suspending fixation model. The recombinant human bone morphogenetic protein - 2 was implanted in the bone tunnel in the experimental group while the operation without the implantation of BMP - 2 was performed in the control group. The specimens were collected 4 weeks, 8 weeks and 12 weeks after surgery. The sections were stained with H.E.and Sirius Red. The special changes of the interface tissue between the bone tunnel and the tendon graft were observed and the information of healing between the tendon and the bone tunnel was analyzed. Results In the BMP - 2 group the abundant penetrating Sharpey fibers appeared at 8 weeks while the Sharpey fibers presented at 12 weeks in the control group. Conclusion The indirect connection healing between tendons and the bone tunnel occurs in the deep portion of the bone tunnel with the suspending fixation methods. The recombinant human bone morphogenetic protein- 2 can accelerates the tendon healing in the bone tunnel.
