摘要
目的分析2型糖尿病(DM)及其大血管病变患者人类白细胞抗原(HLA)-DRB1*04亚型的遗传多态性,探索糖尿病大血管病变的发病机制。方法应用聚合酶链反应-序列特异性引物(polymerasechainreaction-sequencespecificprimer,PCR-SSP)技术对合并/未合并大血管病变的北方汉族2型DM患者的HLA-DRB1*04亚型等位基因进行检测。结果东北地区汉族2型DM患者中共检出9种HLA-DRB1*04亚型。2型DM伴大血管病变组DRB1*0404等位基因频率较2型DM无并发症组显著增高;但DRB1*0403等位基因频率较无并发症组显著减少。结论1型DM与2型DM可能具有相似的HLA遗传背景。DRB1*0404单倍型可能是2型DM患者发生大血管病变的易患基因,DRB1*0403单倍型可能是2型DM患者发生大血管病变的保护性基因。HLA-DRB1*0404等位基因是患免疫相关性疾病患者发生血管内皮功能失调,尤其是大血管病变的预测因子。
Objective Analyzing the polymorphism of HLA - DRB1 * 04 alleles in type 2 diabetics with macrovascular diseases, to explore the pathogenesis of type 2 DM macrovaseular complication. Methods HLA - DRB1 * 04 alleles in type 2 diabetics with or without macrovascular complication were examined by polymerase chain reaction - sequence specific primer ( PCR - SSP). Results Nine genotypes of HLA - DRB1 * 04 alleles in type 2 diabetics were found in Northern - East Hart population. The frequency of HLA - DRB1 * 0404 gene was significantly higher and the frequency of DRB1 * 0403 gene was significantly lower in type 2 diabetics with macrovascular diseases than that without the complication. Conclusion The genetic background of type 2 DM may be similar to that of type 1 DM. The DRB1 * 0404 might be the susceptibility gene of macrovascular diseases in type 2 DM, and the DRB1 * 0403 might be the susceptibility gene preventing type 2 DM from the macrovascular diseases. The HLA - DRB1 * 0404 might be predicting factor to macrovascular diseases.
