摘要
采用45具成人干燥骨枢椎标本测量齿状突外径;采用10具新鲜尸体的寰枢椎骨韧带标本,对直径3.5mm的单枚及双枚加压螺丝钉和自制的齿状突加压螺丝钉就齿状突骨折直接螺丝钉固定的方法进行前、后水平方向剪切刚度、扭转刚度测量.结果:齿状突颈部外径最小冠状径及矢状径分别是8.93mm、10.47mm.三种螺丝钉固定方法都能提供骨折固定后相似的稳定性.研究表明:齿状突骨折后行内固定治疗宜根据CT检查的结果,选用直径4.0~4.5mm的单枚松质骨加压螺丝钉.
The reliability and initial postoperative stability of the dentoid process of axis were evaluated by nondestructive biomechanical testing of ten fresh atlanto-axial joint complex. Type I fracture of the dentoid process of axis were created using an osteotome. Simulated fractures were fixed using one and two 3. 5mm compression screws,and one 4. 5mm compression screw. Intact and three instrumented constructs were tested in torsion,anterior, and posterior shear stiffness. Three direct screw fixation techniques can provide similar stability. Based on the study of axial bone measuring,one 4. 0-4. 5mm cancellou bone compression screw may be the first selection.
