摘要
目的:评价3种鉴定葡萄球菌凝固酶方法的敏感性和特异性。方法:临床分离的110株葡萄球菌,经革兰染色、触酶试验、ARIS2X鉴定后,用试管法、玻片法、商品乳胶血凝法(slidexstaph-kit)和凝固酶基因法,鉴定葡萄球菌和评价凝固酶试验方法。4种方法结果不一致的菌株进行重复试验。用凝固酶基因法作为“金标准”,比较其他3种凝固酶方法的敏感性、特异性。结果:凝固酶基因法能够正确判定葡萄球菌凝固酶;试管法有1株菌鉴定假阴性;玻片法有8株菌假定假阳性,2株假阴性;乳胶血凝法有2株假阳性。试管法、玻片法、乳胶血凝法的敏感性分别为98.53%、97.30%和100%;特异性分别为100%、77.78%和94.44%。玻片法鉴定葡萄球菌凝固酶有9.01%的错误。结论:乳胶血凝法和试管法敏感性和特异性较高,是鉴定葡萄球菌凝固酶试验的好方法,玻片法的特异性较差,可能是由于人血浆中产生的葡萄球菌抗体所致。
Objective Evaluates itg sensitivity and particularity 3 kinds of staphylococci coagulas methods. Methods The method is clinical disjunctive 110 stub staphylococcus, after Gram stain-hucker modification, Catalase test, ARIS2X authentication , using the tube method , the slide agglutination test method , the sidex staph-kit method and coagulase gene method, to authenticate staphylococci and evaluate coagulase test method. The strain will be test again if the result of 4 kind method are inconsistent. As the Coagulasegene method is a "gold standard" , evaluate it's sensitivity and particularity If the 3 kinds of coagulase methods . Results The Coagrlase-gene method can authenticate staphylococci coagulase correctly ; There are 1 strain fake negative result by the tube method; 8 strains fake positive result ,2strain fade negative result by agglutination test method; 2 strains fake positive result . It's sensitivity is 98.53% ,97.30% and 100% ,specificity is 100% ,77.78% and 94.44% correspond with the tube method , the slide agglutination test method, the slidex staphkit method and coagulase gene method. There are 9.01% wrong result by using agglutination test method authenticating staphylococci. Conclusion It's good method to authenticate staphylococci coagulase test by the slidex staph-kit method and the tube method with highly sensitivity and specificity . Otherwise ,the specificity of the slide agglutination test method is lower than the others ,perhaps it caused by staphylococci antibody in human plasm.
