单肺通气时IL-6、IL-8、TNF-α mRNA基因表达和乌司他汀的影响 被引量:12

摘要 目的:探讨化疗后病人单肺通气炎性因子基因表达和乌司他汀的影响。方法:ⅢA期肺癌患者化疗后择期手术30例,Ⅱ-Ⅲ级;随机分为对照组(C组)和乌司他汀组(U组),每组各15例。U组给予乌司他汀10 000 U.kg-1,于麻醉诱导后缓慢静注,C组用等量生理盐水代替。在开胸后和单肺通气90 min后取远离肿瘤的肺组织各1块,逆转录PCR(RT-PCR)检测IL-6、IL-8、TNF-αmRNA的表达。结果:和开胸后相比,单肺通气90 min后两组IL-6、IL-8、TNF-αmRNA的表达均明显增强,但U组单肺通气90 min后IL-6、IL-8、TNF-αmRNA的表达明显受到抑制,和C组比较有显著差异(P<0.05)。结论:化疗后病人单肺通气炎性因子基因表达明显增强,而乌司他汀在转录水平上能抑制炎性因子在肺内的表达,对肺有一定的保护作用。 AIM: To investigate the effects of ulinastatin on expression of IL - 6, IL - 8 and TNF - α mRNA during one lung ventilation. METHODS: Thirty patients with lung cancer ⅢA after chemotherapy ASAⅡ -Ⅲ, selected to pulmonary surgery, were included in this study and were randomly divided into two groups: control group ( group C, n = 15 ) and ulinastatin group (group U, n = 15 ). Patients in group U received ulinastatin (10 000 U · kg^-1 , iv) after induction of anesthesia. Patients in group C: received equal volume of normal saline, instead of ulinastatin. The lung tissue far from the tumor was taken and washed two times by PBS. Total RNA was extracted from the lung tissue using a standard Trizol protocol and expression of IL - 6, IL - 8 and TNF - α mRNA were detected by RT - PCR. RESULTS: The expression of IL - 6, IL - 8 and TNF -α mRNA in two groups were low after induction of anesthesia. They increased significantly 90 rain after one lung ventilation in both two groups. Compared with control group, the expression of IL - 6, IL - 8 and TNF - α mRNA in the ulinastatin group decreased significantly 90 rain after one lung ventilation. CONCLUSION: The expres- sion of IL - 6, IL - 8 and TNF - α mRNA during one lung ventilation increase significantly, and ulinastatin depresse the expression of these cytokines at the transcriptional levels and reduces lung injury.
