摘要
目的:研究抗氧化剂维生素E对维持性血液透析(MHD)患者氧化应激状态的影响。方法:MHD患者58例,口服维生素E400mg/d,服药6周,观察用药前后血浆丙二醛(MDA),循环晚期氧化蛋白产物(AOPP),过氧化物歧化酶(SOD),谷胱苷肽过氧化物酶(GSH-PX)及血浆中维生素E浓度,选20例健康者作正常对照。结果:患者氧化指标AOPP(187.27±54.9μmol/Lvs99.05±26.2μmol/L)和MDA(7.74±2.17nmol/ml vs3.06±1.70nmol/ml)明显高于正常对照组(P<0.01),GSH-PX活性(74.42±27.46μmol/L vs127.27±25.88μmol/L)、SOD活性(70.88±14.74U/ml vs114.67±24.65U/ml)VitE浓度(3.27±1.36μg/ml vs6.58±1.44μg/ml)明显低于正常对照组(P<0.01)及(P<0.05),给予维生素E治疗后,AOPP(143.41±33.7μmol/Lvs187.27±54.9μmol/L)、MDA(5.19±2.85nmol/ml vs7.74±2.17nmol/ml)较治疗前明显降低(P<0.01)、VitE(5.49±2.06μg/ml vs3.27±1.36μg/ml)、SOD活性(108.30±22.95U/ml vs70.88±14.74U/ml),GSH-PX活性(91.56±14.49μmol/L vs74.42±27.46μmol/L)较治疗前升高(P均<0.01)。结论:口服大剂量维生素E(400mg/d)可以改善血透患者的氧化应激状态,血透患者短期应用大剂量维生素E未见明显副作用。
Objective:To investigate the effect of vitamin E supplementation on oxidative stress in patients with MHD(maintenance hemedialysis).Methods:58 patients with MHD and 20 healthy volunteers were involved in this study.The patients were adminis- tered 400mg/d of vitamin E for 6 weeks.The plasma levels of oxidative parameters including vitamin E(VitE),malondialdehyde (MDA), advanced oxidation protein product (AOPP), superoxide dismutase (SOD), activity of glutathione peroxidase (GSH-PX) were measured at the initial and the end of VitE supplementation respectively.Results:Plasma levels of AOPP and MDA ( 187.27 ± 54.9 μmol/L vs99.05 ± 26.2 μmol/L,7.74 ± 2.17nmol/ml vs3.06 ± 1.70nmol/ml)were signiilcanfly higher in patients with MHD compared with healthy controls(P〈0.01).VitE concentration and GSH-PX and SOD activities(3.27 ± 1.36μg/ml vs6.58 ± 1.44μ g/ml,74.42 ±27.46μmol/L vs127.27 ±25.88μmol/L,70.88 ± 14.74U/ml vs114.67 ±24.65U/ml) in MHD patients were signitlcantly lower than those in controls respectively.400mg/d of VitE supplementation decreased the plasma levels of AOPP ( 143.41 ± 33.7 μ mol/L vs187.27 ± 54.9 μ mol/L) ,MDA (5.19 ± 2.85nmol/ml vs7.74 ± 2.17nmoi/ml)CRP ( 3.49 ± 1.34mg/L vs4.71 ± 1.81mg/L)( P〈 0.01 ) ,the levels of VitE(5.49 ± 2.06 p. g/ml vs3.27 ± 1.36 μg/ml) ,SOD( 108.30 ± 22.95U/ml vs70.88 ± 14.74U/ml), GSH-PX (91.56± 14.49p.mol/L vs74.42 ±27.46p.mol/L)were increased (P〈0.01).Conclusion:There are oxidative stress in patients with MHD,400mg/d of VitE ameliorates oxidative stress without apparent side effect.
