摘要
目的:观察封闭负压引流技术(VAC)敷料下压力及其产生的机理和敷料面积大小对它的影响。方法:按照VAC敷料面积的大小分为3组,采用水压法检测不同负压时各组VAC敷料下的压力。在志愿者右前臂前侧贴附VAC敷料,在负压吸引前及-10、-20、-30和-40 kPa吸引后进行CT扫描,测量皮肤距尺桡骨连线的平均距离。结果:采用不同负压吸引时,A组、B组和C组敷料下的压力值分别为5.10-6.78 kPa、3.92-6.60 kPa与2.29-6.13 kPa;负压值越大,敷料下的正压越大。采用相同负压进行VAC治疗时,VAC敷料面积越大,其下的压力越小,在以上各负压值,A组、B组和C组敷料下的正压差异有明显统计学意义(P<0.01)。CT结果显示,不同负压下VAC敷料下皮肤距尺桡骨连线的平均距离与负压吸引前差异无统计学意义(P>0.05)。结论:VAC敷料下正压和负压同时存在,正压随敷料面积的增大而减小,但VAC敷料下的组织不产生明显移位。
Objective:To observe whether positive pressure existed under vacuum -assisted closure (VAC) and the relationship between the positive pressure and the dressing area. Methods: Three groups were set according to the VAC dressing area. The positive pressure under VAC dressing was measured using a specialized device. After a VAC dressing was adhered to the front side of right forear, computed tomography (CT) scan was performed at 0, - 10, -20, and -30 kPa negative pressure under VAC dressing, respectively, and the distance between the skin under the tube and the front side of the ulna was measured. Results: The positive pressures under VAC dressing in Group A, B and C were 5.10 - 6.78 kPa, 3.92 - 6.60 kPa, and 2.29 - 6.13 kPa, respectively. In each group, the positive pressure under VAC dressing increased when the negative pressure was enhanced ( P 〈 0.01 ). When the VAC dressings of different group were exerted with the same negative pressure, the positive pressure under VAC dressing of Group A was stronger than that of Group B, and both of them were stronger than that of Group C (P 〈0.01 ). The figures of CT indicated that the distance between the skin under the tube and the front side of the ulna did not changed when the negative pressure exerted on VAC dressing was enhanced from 0 to -30 kPa. Conclusion: There existed negative pressure and positive pressure under the VAC dressing simultaneously, and under a specific negative pressure, the positive pressure decreased when the VAC dressing area increased. The positive pressure under VAC dressing, offset by negative pressure, did not incur disfiguration of the soft tissue.
