我国正常人眼动脉和视网膜中央动脉血流改变的Meta分析 被引量:8

摘要 目的:分析正常人视网膜中央动脉(CRA)和眼动脉(OA)血流动力学参数的年龄、性别、和眼别差异。方法:计算机网上检索中国期刊1994/2006年来有关CRA和OA彩色超声多普勒血流动力学测定的正常人年龄、性别、和眼别对照研究文献,对14篇文献报道的CRA和OA血流参数进行同质性检验和合并效应量的检验。结果:合并分析后有统计学意义的同质性研究结果为正常老年组的CRA收缩期峰值血流速度、舒张末期血流速度、平均血流速度比中青年组分别降低0.52~0.62,0.35~0.44,和0.56~0.65倍标准差。正常男性的OA收缩期峰值血流速度比正常女性增加0.28~0.33倍标准差,舒张末期血流速度增加0.47~0.52倍标准差,阻力指数降低0.11~0.39倍标准差;CRA收缩期峰值血流速度增加0.23~0.34倍标准差。左眼的OA舒张末期血流速度减慢0.03~0.07倍标准差,搏动指数下降0.03~0.11倍标准差;CRA舒张末期和平均血流速度分别减慢0.19~0.25和0.18~0.29倍标准差。结论:要确定我国正常人的OA和CRA血流动力学参数正常值范围,需要进行多中心大样本的随机对照研究,研究中要标准化检测技术和取样部位,重复检测不同年龄、性别、眼别的血流参数。 AIM: To analyze hemodynamic parameters of ophthalmic artery (OA) and central retinal artery (CRA) in the normal people. METHODS: By using National Knowledge Infrastructure searching, hemedynamic parameters measured with color Doppler ultrasonography from 14 research articles from 1994 to 2006 were cited and analyzed with meta-analysis in homogeneity test and combined test. RESULTS: After the tests, homogeneous research results of the parameters included: (1) men comparing to women: the decrease of peak systolic velocity (0.52-0.62SD), end diastolic velocity (0.35-0.44SD) and mean velocity (0.56-0.65SD) of CRA in the elderly comparing to the young; the increase of peak systolic velocity (0.28-0.33SD), end diastolic velocity (0.47 -0.52SD) of OA; peak systolic velocity (0.23-0.34SD) of CRA; (2) left eyes comparing to right eyes: the decrease of end diastolic velocity (0.03-0.07 SD) and pulsatility index (0.03-0.11SD) of OA as well as end diastolic velocity (0.19-0.25 SD); and mean velocity (0.18-0.29 SD) of CRA. CONCLUSION: To determine the normal ranges of hemodynamics in OA and CRA, a large population-based and multi-centers random study is required.
