摘要
目的观察窦性心率震荡(HRT)的新测量指标———震荡斜率起始时间(TT)及经典测量指标———震荡初始(TO)、震荡斜率(TS)在急性心肌梗死(AMI)患者中的变化,并分析各指标与心率变异性(HRV)时域指标SDNN之间的相关性。方法选择发病1-3周的60例AMI患者和50例年龄、性别匹配的健康人分别作为AMI组和对照组,收集入选者24h动态心电图(Holter)资料,获取HRT和SDNN指标。定量测量HRT包括TO、TS及TT指标,比较两组之间各指标的差异,并分析TT、TO、TS与SDNN之间的相关关系。结果AMI组与对照组HRT各指标及SDNN均存在显著差别(P<0.05),AMI组TO、TT明显高于对照组,TS、SDNN明显低于对照组;且AMI组TS与SDNN间存在一定相关性(P<0.05),对照组各指标与SDNN均无显著相关性(P>0.05)。结论AMI患者TT值增大,提示AMI患者室性早搏后窦性心率减速现象出现时间延迟,结合TT可能会对TO、TS指标进行进一步的危险度分层,TT可能成为HRT的又一项新的高效检测指标。
Objective To investigate the parameter changes of heart rate turbulence (HRT), turbulence timing(TT), turbulence onset(TO) and turbulence slope(TS) in patients with acute myocardial infaretion(AMI), and to analyze the correlation of TT, TO and "IS with SDNN, parameter of heart rate variability(HRV). Methods Sixty patients with AMI for 1 - 3 weeks (AMI group) and 50 healthy controls (control group) were chosen. The 24 h ambulatory dectrocardiograph (Holter) data of subjects were collected to obtain the value of TO,TS, TT and SDNN. The indexes of TT, TO, "IS were determined quantitively and analyzed. Results Each index of HRT and SDNN in two groups showed significant difference(P 〈 0.05). TO and TT were significantly higher in AMI group than in control group, but "IS and SDNN were lower than in control group. "IS was significantly correlated to SDNN in AMI (P 〈 0.05). But there were no correlations between the indexes of HRT and SDNN in control group. Conclusion TT increasing in patients with AMI indicates that the deceleration of heart rate after ventricular premature complexes delays. TT may be another new and effective index of HRT,and can further classify the risk grade of TO and TS.
