糖尿病合并糖尿病足患者血浆肾上腺髓质素的变化

摘要 目的:探讨血浆肾上腺髓质素(ADM)的改变及血浆内皮素(ET)、血管紧张素Ⅱ(AngⅡ)及一氧化氮(NO)的变化与糖尿病合并糖尿病足的关系。方法:糖尿病合并糖尿病足和单纯糖尿病患者各40例(A、B组)与健康对照者30例(对照组)均采用放射免疫分析法测定血浆ADM、ET、AngⅡ的含量,化学比色法测定NO含量,比较血浆ADM含量的改变及与其它因素的相关性。结果:血浆ADM、ET、AngⅡ含量,A组明显低于B组和对照组(P<0.01),血浆NO含量亦明显下降(P<0.05)。A组血浆ADM、ET、AngⅡ略呈正相关。结论:长期高血糖损害血管内皮细胞,使ADM、NO减少,而ET、AngⅡ升高。糖尿病合并糖尿病足时各种血管活性物质都明显减少,在糖尿病足形成发展中发挥不良作用。 Objective. To study the changes of plasma adrenomedullin (ADM) in diabetic patients complicated with diabetic foot (DF) and to study the correlation among plasma ADM with endotheIin (ET), angiotension Ⅱ (Ang Ⅱ ), nitric oxide (NO), Methods: Forty diabetic patients complicated with DF, 40 simple diabetic patients and 30 healthy controls were selected as subjects. Plasma ADM, ET and Ang Ⅱ concentrations were measured by radioimmunoassay and the serum concentration of NO chemical eolorimetry to observe the changes in the level of ADM and their relation to other factors. Results: The plasma levels of ADM, ET and Ang Ⅱ were significantly lower in diabetic patients with DF than in the simple diabetic patients and the normal control subjects (P〈0.01). The serum levels of NO were decreased in the diabetic patients with DF group (P〈0.05). ADM showed a close positive correlation with Ang Ⅱ in plasma (r=0. 248, 0. 329, 0.10〉P〉0. 05 respectively). Conclusion: Hyperglycemia can damage vascular endothelium and reduce the level of ADM and NO, besides, can increase the secretion of ET and Ang Ⅱ. In diabetic mellitus complicated with DF, the increased ET and Ang Ⅱ and the decreased NO can reduce the secretion of ADM. ADM, ET, Ang Ⅱ and NO may play an important and unfavorable role during the development of diabetes mellitus with DF.
