摘要
目的探讨博莱霉素(BLM)致肺纤维化(PF)的影像学表现与血管内皮细胞(EC)病理学间的关系。方法健康SD大鼠随机分为实验组和对照组各20只,实验组大鼠制作BLM损伤PF模型,分别在第3、7、14、28天(各5只)行HRCT扫描后、检测外周血中vWf水平、肺血管造影及制作电镜标本。对照组方法同实验组。结果实验组大鼠外周血vWf水平均高于对照组(P<0.05),以3天组为著(P<0.01)。磨玻璃样影和实变影主要见于3、7天组,支气管血管束增粗见于7天以后。蜂窝肺则见于14、28天组,以28天组为著。实验7、14天组肺动脉远端纹理明显增粗,分支增多,血管周围显示模糊,28天组肺动脉远端纹理纤细,分支减少,近端肺动脉增粗。实验3天组EC变性、坏死,部分区域见EC增生,7、14天组均可见新生毛细血管,分布于靠近基底膜的纤维化区域。结论HRCT能较准确反应PF的病理变化及病程,EC损伤与PF发生、发展过程密切相关。
Objective To explore the correlation between imaging of bleomycin (BLM) induced pulmonary fibrosis (PF) and pathology of microvessel endothelial cells. Methods Forty healthy SD rats were equally divided into control and experiment group in random, there were 20 rats in each group. In both groups, the vWf in peripheral blood were tested, the lung angiography were observed, and the lung samples of electron microscopy were made after the lungs were scanned by HRCT at the 3rd, 7th, 14th and 28th day when BLM induced PF were finished. Results The vWf in peripheral blood of the experi- ment group was higher than that of the control group (P〈0.05), and the highest was at the 3rd day. Ground glass-like imaging and consolidation of the lungs were observed at the 3rd and 7th day, thickening of the bronchus blood vessel was observed at the 7th day, and honeycombed imaging at the 14th and 28th day. At the 7th and 14th day, thickening of the markings of distal end of pulmonary artery, increase of the branches and blurring around the vessel were observed. At the 28th day, tenuity of distal end of pulmonary artery, decrease of the branches, and thickness of proximal end of pulmonary artery were observed. Degeneration and necrosis of EC and hyperplasia in some area were found at the 3th day, and neogenetic vessel were found distributed around the fibrosis area of basal membrane at the 7th, 14th day. Conclusion High-resolution CT can reflect the extent and process of the pathological changes of PF, injury of EC was closely related with genesis and development of PF.
