摘要
目的探讨术前区域性动脉灌注化疗对晚期乳腺癌细胞凋亡及其相关基因表达的影响。方法将56例晚期乳腺癌患者随机分为治疗组(术前动脉灌注化疗)和对照组(未行术前化疗),各28例。利用TUNEL法检测两组的癌细胞凋亡情况,运用免疫组化方法检测两组bcl-2、bax基因的表达状况。结果乳腺癌细胞凋亡指数(AI)治疗组为7.28%±2.68%,对照组为4.21%±1.87%;bcl-2基因的表达,治疗组为0.68±0.06,对照组为2.24±0.36;bax基因的表达,治疗组为0.72±0.06,对照组为0.38±0.04。上述指标两组间比较均有显著性差异(P<0.05)。结论术前区域性动脉灌注化疗能诱导肿瘤细胞凋亡,而术前化疗后肿瘤细胞AI上升与化疗的疗效有关;术前化疗可影响乳腺癌细胞凋亡相关基因的表达,bcl-2/bax在诱导细胞凋亡中起重要作用,可作为预测化疗效果的指标。
Objective To investigate the effect of preoperative regional intra-arterial chemotherapy on apoptosis and apoptosis-regulating genes in advanced breast cancer. Methods 56 patients were divided into 2 groups: Preoperative regional intra-arterial chemotherapy group (treatment group, 28 cases) and non-preoperative chemotherapy group (control group, 28 cases). Apoptotic cells were examined by TUNEL and bcl-2 expression, and hax was detected by immunohistochemical techniques. Results Apoptotic index (AI) of breast cancer in the treatment group was 8. 74%; while that of the control group was 4. 65%. The bcl-2 expression in treatment group was 0. 68±0. 06, while in control group was 2. 24±0. 36. Expression of hax in treatment group was 0. 72±0. 06, while in control group was 0. 38±0. 04 (P〈0. 05). Conclusion Preoperative intra-arterial chemotherapy can induce the apoptosis of cancer cells and affect the expression of bd-2/bax genes in advanced breast cancer. The bcl-2/hax genes may possibly be used to predict the response to treatment in breast cancer patient.
