慢性肾炎患者肾组织蛋白酶B的表达与尿蛋白的关系

摘要 目的探讨慢性肾炎患者肾组织蛋白酶B(cathepsin B,CB)的表达及其与尿蛋白的关系。方法对肾活检组织病理切片进行形态计量,对病变肾小管肾间质进行半定量积分,分为轻度损害组、中度损害组和重度损害组。采用免疫组织化学SP法和计算机图像分析系统检测肾组织CB的表达情况,并检测24h尿蛋白和NAG酶(N-乙酰-β-D氨基葡萄糖苷酶)。结果CB在近曲小管表达最高,在其他部位表达较少。中度损害组和重度损害组CB的表达明显低于轻度损害组(P<0.01)。24h尿蛋白定量和NAG随肾小管损害程度加重逐渐升高,3组相比差异有统计学意义(P<0.05)。CB表达与肾小管肾间质损害积分、24h尿蛋白量和NAG呈显著负相关(r分别为-0.89、-0.23和-0.75,P均<0.01)。结论CB主要在肾近曲小管表达,可能参与了肾小管中尿蛋白的降解。其表达减少,将导致肾小管上皮细胞中蛋白的堆积,加重肾小管间质的损害。 Objective To elucidate the expression of cathepsin B in the patients with chronic glomerular nephitis and the relationship between cathepsin B and 24h urine protein. Methods According to the degree of tubulointerstitial injury, the patients were divided into the light damage group,the midddle damage group and the heavy damage group. The expression of cathepsin B in the renal biopsy specimens was analysed using computer image analysis system on the immunohistochemical stained section. The data of 24h urine protein and n-acetyl-β-d-glucosaminidase(NAG) were collected and the relationship between the expression of cathepsin B and the 24h urine protein was analyzed. Results The cathepsin B expression was strong in renal proximal tubule. The cathepsin B expression was significantly decreased in the middle and heavy damage groups compared with that in the light damage group (P〈0.01). The more the levels of 24h urine protein and NAG, the higher the degree of tubulointerstitial injury. Significant difference of 24h urine protein and NAG wer found in three groups. The cathepsin B expression significantly negatively correlated with the integral calculus of tubulointerstitial injury( r=-0.89, P〈0.01 ), 24h urine protein (r= -0.23 ,P〈0.01 ) and NAG(r= 0.75, P〈0.01). Conclusion The cathepsin B is located in the proximal tubule and it might contribute to the degradation of the protein resorbed by proximal tubule. The decreased expression of cathepsin B results in the over-deposition of protein in renal tubular epithelial cells, which might correlated with the tubulointerstitial injury.
