Bi-PAP呼吸机辅助通气治疗COPD并发呼吸衰竭的临床观察与护理 被引量:11

摘要 目的:观察Bi-PAP呼吸机治疗慢性阻塞性肺疾病(COPD)并发呼吸衰竭的疗效。方法:39例患者按住院先后顺序分为2个实验组,呼吸机组21例,持续Bi-PAP呼吸机治疗;吸氧组18例,持续低流量吸氧。两组同时给予抗炎、平喘、补液等常规治疗并辅以积极的护理措施。观察血气和临床症状变化。结果:治疗7d后,呼吸机组比吸氧组其pH值(△PH呼吸机组0.13±0.02,吸氧组0.11±0.07)、PaCO2(△PaCO2呼吸机组2.51±1.02,吸氧组0.51±2.42);PaO2(△PaO2呼吸机组3.86±2.02,吸氧组1.13±1.77)改善程度差异有显著性(P<0.01)。结论:COPD并发呼吸衰竭患者使用Bi-PAP呼吸机治疗,对低氧血症和CO2潴留改善明显,疗效明确,可提高患者的生存质量。 Objective:To observe the effect of Bi - PAP breath machine treatment chronicity blocking lung disease (COPD) concurrent breath failure curative. Methods:39 example patients successively divide 2 experimental group , breath aircraft crews in proper order 21 examples according to being in hoso pital , sustained breath machine treats; Attract oxygen forming 18 examples, sustained low rate of flow attracts oxygen. Give the measure curing and aso sisting the active nursing to routine such as resisting imflammation , putting down asthma , fluid infusion two set at the same time, Observe courage and uprightness and clinical symptoms change. Results:After 7 days, the breath unit ratio attracts the oxygen group, its PH value ( A pH breath unit 0. 13 ± 0. 02, attracts oxygen group 0.11±0.07 ), PaCO2 ( △ PaCO2 breath unit 2. 51±1. 02, attracts oxygen group 0.51 ± 2.42 ) ; PaO2 (△ PaO2 breath unit 3. 86 ± 2.02, attracts oxygen group 1.13 ±1. 77 ) to improve the degree difference to have significance( P 〈 0.01 ). Conclusion:The COPD concurrent breath failure patient uses the Bi - PAP breath machine treatment, and CO2 keeps the improvement to the low oxygen blood sickness to be obvious, and the effect is clear, may improve patient's survival quality.
