摘要
目的比较激光治疗各区域早产儿视网膜病变(ROP)的临床效果。方法回顾性分析波长810nm的二极管激光治疗的42例(73眼)的临床资料。结果42例患儿中Ⅰ区病变10例19眼(平均胎龄30周),Ⅱ区病变32例54眼(平均胎龄30.33周);治疗时矫正胎龄分别为(37.4±3.89)周和(39.41±6.44)周,Ⅰ区病变与Ⅱ区病变的出生体重、胎龄及治疗时矫正胎龄无统计学意义;Ⅰ区病变2例4眼(4/19,21.05%)进行二次激光,Ⅱ区病变3例5眼(5/54,9.26%)进行再激光;Ⅰ区病变中3例5眼(5/19,26.31%)进展为4期,而Ⅱ区病变的患儿病变全部得到控制;激光时年龄与胎龄负相关而与矫正胎龄正相关。结论二极管激光治疗Ⅰ区病变和Ⅱ区病变是安全有效的;Ⅰ区病变激光治疗的预后较差。激光时年龄与胎龄及矫正胎龄有关。
Objective To compare the treatment efficacy of indirect diode laser for zones Ⅰ and zone Ⅱ of retinopathy of prematurity(ROP). Methods The clinical records in 42 preterm infants (73 eyes) treated in Peking University People' s Hospital from 2002 September to 2004 June were retrospectively analyzed. Various risk factors were compared between the patients with zone Ⅰ ROP and zone Ⅱ ROP. Results Sixty-three eyes with ROP were cured via the laser photocoagulation with the curative rate 93.15%. The mean birth weight, gestational age and the corrected gestational age at the time of operation in 10 infants with zone I disease was 1 307.0 g ± 295.6 g,30 weeks and 37.4 ± 3.89 weeks, and the data above mentioned had not significant difference in comparison with the 32 infants with zone H disease group (1 491.44 g ± 295.94 g,30. 33 weeks and 39.41 ±6.44 weeks). Two (4 eyes) of 19 infants with zone Ⅰ disease underwent the second laser and 3 of 32 infants received the same management in another group. Five eyes of 3 patients in the zone Ⅰ ROP group progressed to stage Ⅳ but all patients with zone H ROP regressed following the laser treatment. The age at surgery showed a positive correlation with corrected gestational weeks of infants ( Pearson = 0. 97, P 〈 0. 01 ) and a negative correlation with gestational age ( Pearson = - 0. 391, P 〈 0. 05). No significant relation was found between the zone of ROP in the following parameters: body weight at birth, gestational week,age at surgery and correct gestational week (P 〉 0. 05) Conclusion Diode laser retinal photocoagulation is a safe and effective procedure for treatment of zone Ⅰ and Ⅱ of ROP. Patients with zone Ⅰ ROP always show a poor outcome. Age at the time of the laser photocoagulation is associated with gestational age and corrected gestational age of infants.
