摘要
目的探讨霉菌性食管炎的数字化X线表现。方法对126例患者采用数字胃肠造影的多相位连续摄片动态检查技术观察食管的充盈、蠕动、粘膜皱襞和排空功能;并根据病变范围和有无溃疡形成,分析X线表现。结果126例霉菌性食管炎的X线表现是轻度27例,仅累及食管一段,可见由假膜斑块形成的小的薄壁的充盈缺损,是最早期的形态学改变,常被误认为是气泡;局部粘膜可增粗、紊乱,管壁柔软,蠕动无减弱;中度62例,累及食管中上段或中下段或全长,可见分泌液,并见絮状物影和多发大小不一的薄壁的充盈缺损,粘液增粗、紊乱、扭曲,可呈结节样、卵石样或圆形改变,无溃疡形成;管壁柔软,可见痉挛波;重度37例,累及食管全长,食管分泌液多,絮状物影明显,可见多发的线条状或不规则的斑块样充盈缺损,其边界稀疏,并且有明显纵向趋势;粘膜明显增粗、紊乱、扭曲,呈结节样、卵石样或圆形改变;可见溃疡形成,其形态不一,较细小;蠕动减弱,排空差,有痉挛波,可有管腔狭窄,但管壁柔软,无僵硬。结论数字化连续摄片这一动态检查技术可提高对霉菌性食管炎数字化X线表现的认识,减少食管钡餐检查时的漏诊和误诊。
Objective:To discuss the digital X-ray findings of mycotic esophagitis. Methods: 126 cases were examined by digital dynamic sequential spot films. We observed esophageal filling, peristalsis, mucosal folds, emptying and base on lesion limits with or no ulcer, analyse the X-ray findings. Results:The characteristhe of 126 cases by X-ray examination: 27 eases with light illness history only involved a section of esophagus, a similar pattern of subtle marginal filling defects due to small pseudomembranous plaques that can be mistaken for tiny air bubbles is the earliest morphology; mucous membrane widened and disordered in esophagus, the wall of the esophagus is soft, peristalsis don't decrease. 62 cases with middle illnesss history involve mid-and-upper esophagus or mid-and low or full length, can see secretions and flocci, and multiple subde marginal falling defects, the filling defects is variability in size, mucous membrane widened and disordered in esophagus, is nodular, cobble-like or round, no ulcer; the wall of the esophagus is soft, spastic wave occunr. 37 cases with heavy illnesss history involove full length, esophageal secretions is much, evident flocci, and multiple linear or irregular plague-like fdliag defects, discrete margins and a predominandy longitudinal: the widness of mucous membrane increased obvously disordered, is nodular, cobble-like or round, formed light ulcer, the ulcer is variabiloby in size; peristalsis decrease, the empty of the esophagus is slow, spastic wave occurs. Esophagus is narrow, but the wall of the esophagus was soft, non stiffenss. Conclusion: As the dynamoic monitoring technique, digital sequentiall spot films can improve our cognition on digital X-ray findings of mycotic esophagitis, decrease the misdiagnosis and undiagnosis of esophageal barium neal examination.
