特发性脊髓疝的诊断与治疗

摘要 脊髓疝（spinal cord herniation，SCH）是临床少见病．可分为特发性（idiopathic）、创伤后性（posttraumatic）和医源性（iatrogenic）三类。1974年Wortzman等报道了第1例特发性脊髓疝（ISCH）。截止2006年10月英文文献报道的ISCH已有80余例。ISCH的发生过程包括两个步骤，首先是脊髓腹侧面的硬膜存在先天发育缺陷或者继发于后天慢性损伤，出现微小的局部缺损，形成病理基础；然后，随呼吸和心跳形成的脊髓搏动，在胸椎后凸的存在下，受力点集中于硬膜囊腹侧面．长时间作用下脊髓通过硬膜缺损处向前或前侧方疝出，出现胸段脊髓压迫症的表现。
