摘要
目的探讨重叠综合征(OS)夜间睡眠、呼吸紊乱及夜间低氧的情况,并了解双水平正压通气(BiPAP)的疗效。方法经全夜多导睡眠图(PSG)监测将慢性阻塞性肺疾病(COPD)稳定期患者分出OS组和COPD组,对19例0S患者进行夜间双水平正压通气治疗,对治疗前、后的睡眠呼吸参数进行分析。结果OS组呼吸紊乱指数(RDI)、呼吸紊乱持续最长时间(DBLon)、氧饱和度低于90%的时间占睡眠时间的百分比(SLT90)显著高于COPD组(P〈0.01),觉醒次数(AI)、浅睡眠占全夜总睡眠时间的比例(S1+2/TST)高于COPD组(P〈0.05);夜间最低氧饱和度(SaO2L)显著低于COPD组(P〈0.01),深睡眠占全夜总睡眠时间的比例(S件。/TST)、快动眼睡眠占全夜总睡眠时间的比例(REM/TST)、夜间基础氧饱和度(SaO2B)、睡眠效率(SE)均低于COPD组(P〈0.05)。OS组经BiPAP治疗后,上述睡眠呼吸指标改善,而且总的睡眠时间增加,与治疗前相比差异有统计学意义(P〈0.05)。结论OS组患者存在严重的夜间睡眠、呼吸紊乱及夜间低氧,BiPAP治疗对其有效。
Objective To investigate nocturnal sleep,breathing disorder ,hypoxemia and the effects of biphasie positive airway pressure (BiPAP) in the patients with overlapping syndrome (OS). Methods Overnight sleep studies were performed by polysomnography(PSG)in the patients with stable chronic obstructive pulmonary disease(COPD), nineteen patients with OS, eleven eoses with simple COPD were diagnosed by PSG. The patients with OS were treated with BiPAP. The parameters of overnight PSG were compared between two groups before and after treatment. Results The parameters such as RDI, DBLon, SLT90 were significantly higher ( P 〈 0.01), AI, S1+2/TST were higher ( P 〈 0.05); SaO2L was significantly lower ( P 〈0.01 ), S3+4/TST, REM/TST, SaO2 B, SE were lower ( P 〈 0.05 ) in OS patients. The parameters of overnight PSG were improved and TST increased( P 〈0.05) also after the patients with OS were treated with BiPAP during overnight sleep. Condusions The patients with OS were more severe than those with simple COPD on nocturnal sleep, breathing disorder and hypoxemia (P 〈 0.05). The parameters were improved after treated by BiPAP ( P 〈0.05). The patients with OS have more nocturnal sleep,breathing disorder and hypoxemia when they are sleeping, Obviously the treatment of BiPAP is effective.
