摘要
目的比较不同状态下纤维光导喉镜引导气管插管用于阻塞性睡眠呼吸暂停综合征患者的效果。方法根据气管插管时患者状态不同将60例阻塞性睡眠呼吸暂停综合征患者随机分为三组，A组清醒状态下气管插管。B组半清醒状态下气管插管。c组浅全麻状态下气管插管。观察插管效果。结果三组插管成功率分别为70％（14／20）、100％（20／20）、80％（16／20），B组插管成功率明显高于其它各组（P〈0．05）；三组插管时间分别为（4．2±3．1）min、（2．5±1．8）min、（6．3±4．8）min，B组插管时间明显缩短（P〈0．05）。结论半清醒状态下纤维光导喉镜引导气管内插管用于阻塞性睡眠呼吸暂停综合征患者插管成功率曼高，插管时间更短，插管更安全。
Objective To compare the successful rate and time of intubation during fibroptic enocracheal intubation in patients with OSAS at semi-concious; conscious or shallow general anesthesia state. Methods During encracheal intubation, 60 patients with OSAS who are going to operate uvulopalatopharyngoplasty were randomly divided into 3 groups according to different state. Group A： After surface anesthesia, 20 patients were preformed fibroptic enocracheal intubation on conscious state. Group B： After surface anesthesia, 20 patients were performed fibroptic enocracheal intubation on semi-conscious state. Group C： After surface anesthesia, 20 patients were preformed fibroptic enocracheal intubation on shallow general anesthesia state. Times, duration time, and suffessful rate of intubation were recorded. Results 14 intubations were successfully finished in 20 patients of group A, successful rate 70% （14/20）, 11 intubations were successfully finished in the first time. 3 intubations were success- fully finished twice or more, average duration of intubation was （4.2 ~ 3.1）min. Other 6 patient is preformed intubation successfully by way of Group B. And intubation was successfully finished in the first time in all patients of Group B, successful rate 100% （20/20）, average duration of intubaton was （2.5 ± 1.8）min. 16 intubations were successfully finished in 20 patients of group C, successful rate 80% （16/20）, 9 intubations were successfully finished in the first time. 7 intubations were finished after twice or more, average needed time was （6.3 ± 4.8）min. Conclusion Comparing on conscious state and shallow general anesthesia state, intubation have higher successrate, shorter duration and is more secure during fibroptic enocracheal intubation in patients with OSAS on semi-concious state. And patiens did not remenber the performing of intubation and is stress-free. So fibroptic endocracheal intubation in semi-concious state in patients with OSAS have high applicability.
