64排CT脑动脉成像重建技术的评价研究 被引量:6

EVALUATION OF RECONSTRUCTION TECHNIQUES OF 64 MULTI-DETECTOR CT CEREBRAL ANGIOGRAPHY

摘要 [目的]评价64排CT脑血管成像几种重建技术的临床应用价值。[方法]采用GE LightSpeed 64排CT机对怀疑脑血管病变的90例患者行CTA检查,将原始数据导入ADW 4.2工作站利用MPR、MIP、CPR、VR和CTVE进行脑动脉成像,并对图像进行分析。[结果]64排CTA能够清晰显示1~5级脑动脉结构,几种重建技术各有优势。CT脑血管成像发现动脉瘤16例,动静脉畸形13例,脑血管狭窄12例,脑膜瘤10例,正常者39例,病变患者均得到DSA或手术证实。VR对观测脑动脉瘤的部位、大小及其与毗邻结构的关系最为直观准确,在对动静脉畸形的大小、形态以及供血动脉、引流静脉的数量判断上以MIP和VR准确,而CPR和MIP更适于对脑血管狭窄程度的观测。[结论]各种重建技术均可用于诊断脑血管病变,它们的联合应用可提供更多的诊断信息,有助于临床治疗方案的选择。 [Objective] To evaluate the value of several kinds of reconstruction techniques of 64 multi-detector CT cerebral angiagraphy in clinical practice. [Methods] CT angiography was performed in 90 patients with suspected cerebral vascular disorder by a GE LightSpeed 64 multi-detector CT scanner. Then the row data was transmitted into the ADW 4.2 work station, source images were processed by post-processing software of MPR, MIP, CPR, VR, CTVE. All of images were analysed by the two radiologists. [Results] The CTA clearly demonstrated 1-5 grade of the cerebral arteries structures. The different reconstruction technique has its own advantage. All cases diagnosed by 64 multi-detector CT cerebral angiography were confirmed by DSA or operation, including brain aneurysm (16 cases), arteriovenous malformation (13 cases), vessel stenosis (12 cases), meningeoma (10 cases), and normal individual (39 cases) . VR images well displayed the size, shape of brain aneurysm, and the stereoscopic relationship between aneurysm with adjacent structures. The size, shape, feeding arteries and draining veins of AVM could be clearly demonstrated on MIP and VR images. The CPR and MIP had more advantages on measuring vessel stenosis. [Conclusions] Several kinds of reconstruction techniques can all show the brain vascular disorder, their associated application could provide more diagnosis information which are useful for making treatment plans.
