兔健眼角膜缘干细胞取材安全范围的实验研究 被引量:5

Experimental study on safety margin of harvesting limbal stem cells from the healthy eyes in rabbit

摘要 目的确定自体角膜缘干细胞移植健眼取材的安全范围,为临床治疗提供实验依据。方法兔眼角膜按30°、60°、90°等依次递增至210°分组切取角膜缘干细胞组织,术后不同时间点裂隙灯下动态观察每组角膜混浊、角膜上皮染色、新生血管3项临床指标,角膜上皮印迹细胞学检查及HE染色观察角膜上皮的细胞表型和病理学改变。结果兔眼角膜缘切取范围大于150°时,出现角膜混浊,上皮持续缺损及新生血管长入,临床指标评分比较,差异有统计学意义(P<0.01)。印迹细胞学检查显示PAS阳性的结膜细胞。组织病理学可见角膜缘上皮细胞层次减少和新生血管。结论自体角膜缘干细胞移植健眼的取材范围≤150°(相当于5个钟点位)是安全的。切取范围>150°时角膜缘干细胞可能出现不可逆的失代偿。印迹细胞学检查是诊断角膜缘干细胞缺乏简单有效的方法。 Objective To explore the safety margin of harvesting limbal stem cells autograft from fellow eye. Methods The corneas of lagophthalmus divided into 7 groups according to the angle of harvesting ranging from 30° to 210° at stepwise increment of 30°. The three clinical indexes including opacity, fluorescein staining, neovascularization of corneas were observed at the end of 1 ^st, week, 2^nd week and a month postoperatively, the imprint cytology of corneal epithelium and HE staining were used to observe phaenotype and pathologic changes of corneal epithelial cells. Results When the angle of harvesting limbal stem cells autograft exceeded 150°, the corneal opacity, persistent corneal epithelial defect, corneal new vessels started to emerge (Rank test, P 〈0. 01 ). The imprint cytology showed the corneal cells was positive for PAS staining. The decreased layers of limbal epithelial cells and corneal new vessels could be manifested by histopathologic examinations. Conclusion The safety margin of harvesting limbal stem cells autograft was smaller than or equaled to 150°. If the angle exceeded 150°, the decompensations of limbal stem cells could not be corrected. The imprint cytology was a simple and effective method to diagnose the defect of limbal stem cells.
