血清FFA、TNF-α与2型糖尿病患者大血管病变的相关性研究 被引量:8

摘要 目的:探讨游离脂肪酸、肿瘤坏死因子-α与糖尿病大血管病变的关系。方法:60例2型糖尿病患者,根据是否合并大血管病变,分为非大血管病变组29例和大血管病变组31例,与28名正常对照组比较,采用比色法测血清游离脂肪酸,ELISA法测血清肿瘤坏死因子-α,对大血管病变的危险因素行Logistic回归分析。结果:2型糖尿病组较正常对照组血清游离脂肪酸、肿瘤坏死因子-α显著增高(P<0.01),大血管病变组血清游离脂肪酸、肿瘤坏死因子-α与非大血管病变组、正常对照组比较,差异有统计学意义(P<0.01)。2型糖尿病组为整体,有无大血管病变为因变量Y(有=1,无=0),以肿瘤坏死因子-α、游离脂肪酸及其他危险因素为自变量进行Logistic回归分析,年龄、病程、收缩压、肿瘤坏死因子-α、游离脂肪酸进入回归方程。结论:游离脂肪酸、肿瘤坏死因子-α是2型糖尿病大血管病变的重要的危险因素。 Objective To study the relationship between free fatty acids, tumor necrosis factor-α and type 2 diabetic patients with macrovascular complications. Methods Sixty cases of type 2 diabetes mellitus among which 31 patients suffered from macrovascular complications and 29 did not, were compared with the normal controls. The level of serum free fatty acids was measured with colorimetric method and tumor necrosis factor-α was measured with enzyme linked immunosorbent assay. Serum free fatty acids, tumor necrosis factor-α and other factors related to diabetic macrovascular complications were analyzed with multiple stepwise logistic regressive analysis. Results Serum free fatty acids, tumor necrosis factor-α were higher in type 2 diabetes mellitus group than those in the normal controls (P〈0. 01). Serum free fatty acids, tumor necrosis factor-α in diabetic macrovascular complications group were higher than those in the non-diabetic macrovascular complications group as well as the normal controls (P 〈 0. 01 ). The multiple stepwise logistic regression analysis was used to define the association between serum free fatty acids, tumor necrosis factor-α and macrovascular complications. Age, systolic hypertension, duration, serum tumor necrosis factor-α, free fatty acids were related significantly with diabetic macrovascular complications. Conclusion Serum free fatty acids and tumor necrosis factor-α are important risk factors of diabetic macrovascular complications.
