摘要
【目的】探讨先天性心脏病(congenital heart disease,CHD)超声彩色多普勒显像(CDFI)诊断与手术对比的符合率。【方法】彩色多普勒超声诊断378例先心病,并经心血管造影及/或手术证实。【结果】本组378例先心病中,完全诊断符合率94.2%(356/378),部分诊断符合率5.8%(22/378)。(1)单纯性先心病255例,完全诊断符合率95.3%,部分诊断符合率4.7%。(2)复合先心病(两种或两种以上单纯畸形并存)31例,完全诊断符合率90.3%,部分诊断符合率9.7%。(3)复杂先心病92例,完全诊断符合率91.3%,部分诊断符合率8.7%。【结论】CDFI检查先心病时,要注意单纯先心病其分型失误,及复合与复杂先心病合并多种畸形的诊断与鉴别诊断,防止漏诊。彩色多普勒技术,为临床提供了可靠的手术依据,应作为先心病诊断最重要的首选方法。
[Objective] To explore the value of color Doppler flow imaging in the diagnosis of congenital heart disease. [Methods] A total of 378 cases of congenital heart disease diagnosed by CDFI and approved by angiocardiography or operation were retrospectively analyzed. [Results] In this group of 378 cases, the rate of complete accordance was 94.2% （356/378）, partial accordance was 5.8% （22/378）. In 255 cases of simple congenital heart disease, complete accordance rate was 95.3%, partial accordance rate was 4.7%. In 31 cases of compound congenital heart disease （2 and more than 2 simple abnormalities） complete accordance rate was 90.3% and partial accordance rate was 9.7%. In 92 cases of complex congenital heart disease, complete accordonce rate was 91.3% and partial accordonce rate was 8.7%. [Conclusions] When diagnosing congenital heart disease by CDFI, more attention shoule be paid to differentiate different types of congenital heart disease to avoid faults and missing. The technique of CDFI provideas credible evidence for clinic operation and is a preferred method in the diagnoses of congenital heart disease.
