胆红素水平与冠状动脉狭窄严重程度的关系

摘要 目的:探讨胆红素水平与冠心病患者冠状动脉狭窄程度及范围的关系。方法:选择接受冠状动脉造影的患者504例,根据冠状动脉造影结果将冠状动脉狭窄者分为冠心病组,无狭窄者为对照组,所有患者均空腹抽静脉血测定总胆红素、胆固醇、甘油三酯、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇水平。冠状动脉的狭窄程度用造影图像处理系统测量,再根据病变损害的程度评分。用统计学方法分析控制其他易患因素影响后血浆总胆红素水平与冠心病的定性关系和总胆红素水平与冠状动脉狭窄程度及范围的定量关系。结果:冠心病组患者387例血浆总胆红素水平(11.57±4.68)μmol/L低于对照组117例(15.09±5.66)μmol/L,差异有显著性(P<0.0001),偏相关分析发现,低血浆胆红素水平对冠心病危险性有独立的预报价值。多元回归分析发现,血浆总胆红素水平与冠状动脉狭窄的病变程度呈负相关(P<0.00001)。结论:胆红素水平与冠心病发生的危险性及冠状动脉硬化程度呈负相关,胆红素水平可作为预测冠心病严重程度的参考指标之一。 Objective:To investigate the relationship between the plasma total bilirubin levels and severity of coronary artery stenosis. Methods :504 patients who underwent coronary angiography were included in the study and divided into two groups according to severity of coronary artery stenosis: Patients with coronary stenosis were include in the group of coronary artery disease (CAD) and patients with normal coronary artery were used as controls. Plasma total bilirubin, total cholesterol (TC) , triglycerides (TG) , lower density lipoprotein cholesterol (LDL- C) , high density lipoprotein cholesterol (HDL- C ) were determined in all patients. Coronary atery stenosis was measured by angiographic imaging manipulation system. The severity of CAD was quantified in a modified Gensini score. After the influences from other risk factions of CAD were controlled by statistical Method, the relationship between plasma total bilirubin levels and severity of CAD was analyzed. Results: Plasma total bilirubin in patients ( 11.57± 4. 68μmol/L) with CAD is statistically significant lower than those in the controls ( 15.09 ± 5.66 μmol/L, P 〈 0.0001 ) , Total bilirubin levels inversely associated with the risk of CAD by analysis of Partial correlation analysis, Multiple regression analysis indicated that plasma total bilirubin levels was inversely correlated with the severity of CAD ( P 〈 0.00001 ). Conclusions: The above results suggest that the plasma total bilirubin levels is inversely associated with the prevalence and severity of CAD, it may be one of the predictors for severity of CAD.
