摘要
目的:综述肝组织工程研究概况,并详细讨论了肝组织工程的影响因素,例如种子细胞的如何选择,支架材料的形态最佳化,复杂的血管化和免疫等,以及对将来的研究方向作出展望。资料来源:应用计算机检索Elsevier SDOS 2000-01/2005-12有关肝组织工程的文章,检索词“liver-tissue engineering,scaffold,hepatocyte”,限定文章语言种类为English。同时计算机检索中国期刊全文数据库、万方数据2000-01/2005-12期间的相关文章,检索词“肝组织工程,支架材料,肝细胞”,限定文章语言种类为中文。资料选择:对资料进行初审,选取符合研究要求的有关文章找全文。纳入标准:①有关肝组织工程研究。②有关肝支架材料以及表面修饰的研究性论文。③有关肝组织工程血管化的研究。排除标准:相关度不大的文章,重复性的文章。资料提炼:共收集到101篇涉及到肝组织工程的文章,其中包括种子选择,支架材料选择和修饰,血管化等因素的文章30篇符合要求。资料综合:①肝组织工程的背景:肝组织工程是解决肝癌系列肝疾病的最佳途径,也是最有潜力的方法之一。②肝组织工程种子细胞的选择:骨髓源干细胞,肝卵圆细胞,原代肝细胞,胚胎肝细胞,肝细胞株。③肝组织工程支架材料的选择和改性修饰:支架材料分两大种类,一种是天然基质材料,一种是合成高分子聚合物,分别叙述了其优缺点,而通过改性修饰(包括复合改性、蛋白质修饰、多肽修饰、氨基酸修饰、尺寸和形状特征修饰)来进一步完善和增殖。④血管化问题:血管化有两种生长方式:血管长入血管化-生长成型;体外制造血管化-制成成型。最近的研究热点集中于体外制造血管化-制成成型。⑤免疫问题以及展望:肝组织工程的高度复杂性使得其研究位于一个初步的阶段,相信随着科学的不断进步,所有的问题都可以得到一一解决,实现最终的真正意义上的新肝脏。结论:肝组织工程虽然取得了初步的成效,但由于其构造以及生长过程的复杂性,所以其研究还面临着诸多问题,但是相信随着科学技术的进步,所有的问题包括细胞增殖问题,材料最佳化问题,血管化和免疫问题都能得到一一的解决,实现真正的肝组织工程。
OBJECTIVE: To illustrate the study on the liver tissue engineering and discuss particularly the influencing factors of the liver tissue engineering including the choice of cell resource, the optimization of scaffold and the vascularization and immunity problems and look ahead the research direction of liver tissue engineering.
DATA SOURCES: A computer-based online search of Elsevier SDOS was undertaken to identify the articles about liver tissue engineering published in English between January 2000 and December 2005 with the key words of "liver-tissue engineering, scaffold, hepatocyte". Meanwhile, we searched China Periodicals Full-text Database and Wanfang database for the related Chinese articles published between January 2000 and December 2005 with the same key words in Chinese.
STUDY SELECTION: The data were firstly selected to choose those met the requirements and look up the full texts. Inclusive criteria: (1)overviews related to liver tissue engineering; (2)study papers about the choice and modification of scaffold; (3)researches about the vascularization problems in liver tissue engineering. Exclusive criteria: the irrelevant and repeated articles.
DATA EXTRACTION: 101 articles referring to the liver tissue engineering were collected and 30 met the inclusive criteria involving the choice of cell resource, choice and modification of scaffold and the vascularization.
DATA SYNTHESIS: (1)Background of liver tissue engineering: Liver tissue engineering is the optimal approach and also one of the most potential methods to cure the liver diseases such as liver cancer et al. (2)Choice of cell resource: Bone marrow-derived stem cells, hepatic oval cells, primary hepatocytes, embryonic hepatocytes and cell lines. (3)Choice and modification of the scaffold: There are two kinds of scaffold including natural extracellular matrix material and synthetic high molecular polymer material, which have advantages and disadvantages and are improved through the modification of recombination, protein, peptides of various lengths, carbohydrates and dimension and morphology. (4)Vasculadzation: There are two kinds of growth patterns for vascularization such as angiogenesis to occur into the cell aggregate and pre-establishment of a vascularized bed to seed the cells around the network. Recent study attention focuses on the latter. (5)Immune response considerations and outlook: Because of its high complex nature, liver tissue engineering still has many problems to solve. With the development of sciences, it is hopeful to obtain the entire implantable new organ after all the problems are resolved.
CONCLUSION: Though the liver tissue engineering has achieved some goals, there are many problems to solve because of the high complexity of its structure and growth. But with the development of the technology, all the problems including cell proliferation, the optimization of scaffold and vascularization and immunity would be resolved to realize real liver tissue engineering.
