摘要
目的探讨多囊卵巢综合征（PCOS）与胰岛素抵抗的相关性研究。方法选择健康妇女及患者各20例。按体重指数分Ⅰ组（正常非肥胖组）、Ⅱ组（正常肥胖组）、Ⅲ组（PCOS非肥胖组）、Ⅳ组（PCOS肥胖组）。比较四组患者性激素浓度、体重指数、腰臀围比、OGTT试验及胰岛素释放，稳态模型胰岛素抵抗指数。结果①Ⅲ组、Ⅳ组性激素水平与Ⅰ组、Ⅱ组相比，差异有统计学意义（P〈0．05）。②PCOS肥胖组（Ⅳ组）的睾酮和体重指数、腰臀围（WHR）水平相比（Ⅲ组）差异有统计学意义（P〈0．05），而黄体生成素水平比对照组低（P〈0．05）；③稳态模型胰岛素抵抗指数与体重指数、腰臀围水平比、空腹胰岛素浓度均呈显著正相关。④Ⅳ组1h、2h、3h血糖明显升高，Ⅳ组胰岛素释放量均高于Ⅰ组、Ⅱ组、Ⅲ组（P〈0．05）。结论PCOS患者睾酮升高者同时存在胰岛素抵抗和代谢异常，肥胖能加重PCOS患者胰岛素抵抗。
Objective To explore investigate the clinical characteristics of polycystic ovary syndrome （PCOS） with insulin resistance. Methods 20 patients with PCOS were compared with 20 healthy women .According to body mass index BMI ,they were divided into four groups randoml： group Ⅰ （normal non-fatty group） and group Ⅱ（normal fatty group）, group Ⅲ（non-fatty patient＇ s group） and group Ⅳ （fatty patient＇ s group） .Comparisons were conducted in the sreum LH, FSH. E2, T, BMI ,waist/hip ratio（WHR）,OGTT test and insulin release and index of stable IR. by, respectively. Results C）There were significant differences between PCOS and control group in four kinds of sex hormone levels （P 〈0. 05）.（2）There were significant differences between PCOS with groupⅣand group Ⅲ in T of sex hormone levels, BMI ,waist/hip ratio（WHR）.（3）The index of stable IR was obviously and positively related to the BMI ,waist/hip ratio（WHR）and fasting insulin level （P〈 0. 01）.（4）Serum glucose levelof PCOS group increased significantly within 1h,2h. 3h and insulin relerse levels were observed in each group. While differences in 0h. 1h. 2h and 3h insulin releas.e levels were observed （P〈0. 05）,Conelusion Insulin resistance and insulin metabolic abnormal ist in POCS patients with .Obesity may exacerbate insulin resistance.
