喉癌癌周毛细淋巴管形态学特征与颈淋巴结转移

Correlation between morphological features of lymphatic capillaries and cervical lymph node metastasis in laryngeal carcinoma

摘要 目的研究喉癌癌周毛细淋巴管形态结构特点及密度与喉癌颈淋巴结转移的关系。方法采用免疫组化法、全自动图像分析仪及透射电镜观察,对75例喉癌及癌周组织、8例正常喉组织毛细淋巴管形态结构特点及密度进行了观察和定量研究。结果各型喉癌癌周毛细淋巴管密度、管径及管腔面积均较正常组织明显增高,伴有颈淋巴结转移组较无转移组亦明显增高;喉癌声门上型癌周毛细淋巴管密度与声门下型无显著差异,而二者均显著高于声门型。电镜观察示正常喉组织毛细淋巴管具有较高通透性,喉癌癌周毛细淋巴管通透性更高,呈不同程度扩张及变性破坏改变,内皮细胞连接开放率较正常组织明显增高。结论喉癌颈淋巴结转移与癌周毛细淋巴管形态结构特点及密度密切相关,为探讨喉癌颈淋巴结转移的过程、机制以及防治提供了理论依据。 OBJECTIVE To study the correlation about morphology, structure, density of lymphatic capillary in pericancerous tissue and cervical lymph node metastasis in laryngeal carcinoma. METHODS Immunohistochemistry method, automatic image analysis technology and transmission electron microscope were applied to observe the morphology, structure, density of lymphatic capillary in 75 laryngeal carcinoma tissue, 75 pericancerous tissue and 8 normal laryngeal tissue samples. RESULTS The density, diameter and area of lymphatic capillary in pericancerous tissue were obviously higher than that in normal laryngeal tissue. The density, diameter and area of lymphatic capillary in pericancerous tissue with cervical lymph node metastasis were obviously higher than that without cervical lymph node metastasis. The density of lymphatic capillary was obviously higher in supraglottic and subglottic carcinoma tissue than that in glottic carcinoma tissue, but the diameter and area of lymphatic capillary in carcinoma tissue among the three parts had no difference. The results of electron microscope also showed that lymphatic capillary in normal tissue have high permeability, the lymphatic capillary in pericancerous tissue have more permeability than that in normal tissue, and have expansion and degeneration with different degree. A large amount of endothelial cell junction was opened in pericancerous tissue, which was higher than that in normal tissue. CONCLUSION Cervical lymph node metastasis of laryngeal carcinoma is related with the morphology, structure, density of lymphatic capillary in pedcancerous tissue. This research offers some theory basis for laryngeal carcinoma metastasis process, mechanism and prevention.
