摘要
目的探讨脑脊液β-淀粉样蛋白(Aβ1~42)、tau蛋白和磷酸化tau蛋白诊断阿尔茨海默病与血管性痴呆的敏感性和特异性。方法采用酶联免疫吸附法检测阿尔茨海默病与血管性痴呆患者脑脊液中Aβ1~42、tau蛋白和磷酸化tau蛋白浓度的变化,根据受试者工作特征曲线观察脑脊液中这3种生物学标志物用于诊断阿尔茨海默病与血管性痴呆的敏感性和特异性;采用单因素方差分析以及受试者工作特征曲线对所获结果进行统计学分析。结果阿尔茨海默病组患者脑脊液Aβ1~42浓度低于对照组(P=0.010),tau蛋白浓度高于对照组(P=0.001)和血管性痴呆组(P=0.030),磷酸化tau蛋白浓度高于对照组(P=0.004)。脑脊液Aβ1~42、tau蛋白和磷酸化tau蛋白用于诊断阿尔茨海默病的敏感度为60.00%~96.70%,特异度可达70.00%~90.00%。Aβ1~42、tau蛋白和磷酸化tau蛋白联合检测可提高阿尔茨海默病与血管性痴呆鉴别诊断的敏感性和特异性,敏感度可达86.57%,特异度为90.00%。结论脑脊液Aβ1~42、tau蛋白和磷酸化tau蛋白浓度的变化,不仅对诊断阿尔茨海默病具有较高的敏感性和特异性,而且亦可作为阿尔茨海默病与血管性痴呆鉴别诊断的生物学指标。
Objective To explore the sensitivity and specificity of cerebrospinal fluid concentration of beta-amyloid (Aβ1-42, tau protein and phosphor-tan (Ptau) protein for the diagnosis of Alzheimer disease (AD) and vascualr dementia (VaD). Methods Enzyme-linked immunosorbent assay was used to determine the cerebrospinal fluid concentration of Aβ1-42, tan protein and Ptau protein in patients with AD and VaD. The sensitivity and specificity of the 3 biomarkers in the diagnosis of AD and VaD were shown in the receiver operating characteristic (ROC) curve. Statistical methods include one-way ANOVA and ROC curve. Results The cerebrospinal fluid Aβ1-42 concentration in AD group was lower than that in control group (P= 0.010). In AD group, the concentration of tan protein was higher than that in control group (P= 0.001) and VaD group (P =0.030), and the concentration of Ptau protein was higher than that in control group (P=0.004). In the diagnosis of AD the sensivity and specificity of cerebrospinal fluid Aβ1-42, tau protein and Ptau protein were 60.00%-96.70% and 70.00%-90.00%, respectively. If combining Aβ1-42, tan protein and Ptau protein to differentiate AD and VaD, the sensitivity and specificity can achieve to 86.57% and 90.00% respectively. Conclusion Cerebrospinal fluid Aβ1-42, tan protein and Ptau protein are useful biomarkers for the diagnosis of AD with high sensitivity and specificity, and can be used to differentiate AD and VaD.
