摘要
目的探讨深部真菌感染小鼠血浆(1→3)-β-D-葡聚糖水平与抗真菌药物疗效的关系。方法110只ICR小鼠随机分成3组,空白对照组、模型组、氟康唑组,建立深部白色念珠菌感染的动物模型。应用MB-80微生物动态快速检测系统及GKT-5MSet动态真菌检测试剂盒定量检测血浆中(1→3)-β-D-葡聚糖的含量,双肾组织匀浆液培养计数菌落数。结果①造模后24小时模型组菌落计数与血浆(1→3)-β-D-葡聚糖的水平比空白对照组明显升高,差异具有显著性(P<0.05)造模成功。②氟康唑组菌落计数与血浆(1→3)-β-D-葡聚糖水平随着治疗逐渐减少,与模型组比较差异具有统计学意义(P<0.05)。③模型组各时间点及氟康唑组治疗前、治疗第1、3、7天菌落计数与血浆(1→3)-β-D-葡聚糖水平具有线性相关关系(P<0.05)。结论检测血浆(1→3)-β-D-葡聚糖是一种实用的深部真菌感染早期快速的诊断方法,(1→3)-β-D-葡聚糖的水平与抗真菌药物疗效呈线性相关,可以借助连续监测血浆(1→3)-β-D-葡聚糖的水平评估药物疗效。
Objective This study was done to evaluate the relationship between the level of plasma( 1→3 ) - β - D - glucan and the therapeutic effect on systemic candidiasis in mice. Methods One hundred and ten ICR mice were divided into three groups at random: normal control group ( 10 ) ,model group (50), and fluconazole group (50). Mice were infected by injection of Candida albicans into the lateral tail vein except norreal control group. Treatment was begun 24h post - challenge. Every 10 mice of the model group and fluconazole group were randomly sacrificed at pretherapy, and the 1 st day,3 rd day,5th day and 7th day of treatment. Content of plasma( 1→3 ) - β -D -glucan were measured by Microbiology Kinetic Rapid Reader MB - 80. Two kidneys were aseptically removed and homogenized in 5 ml of sterile saline. The plates were incubated for 48 h at 37℃ before quantitating Candida growth by colony counts. Results ①The kidney colony count and plasma( 1→3 ) - β - D - glucan of the model group were heightened 24h after injection compared to those of the normal control group. The differences were significant( p 〈 0.05 ). It indicated that the model established successfully and the increased abnormally of plasma( 1→3 ) -β -D -glucan could reflect the existence of sys- temic candidiasis. ②The kidney colony count and plasma( 1→3 ) - β - D - glucan of the fluconazole group were decreased gradually along with treatment. The differences were significant between fluconazole group and model group in kidney colony count and plasma( 1→3 ) - β -D -glucan level( P 〈0.05). ③The kidney colony count and plasma( 1→3 ) -β -D- glucan level of each time point of the model group and those of the pretherapy ,1 st day, 3rd day,Tth day of the fluconazole group shown a perfect linear correlation ( P 〈 0.05 ). Conclusion Plasma( 1→3 ) - β - D - glucan measurement is a practicable method in early diagnosis of systemic candidiasis, continuous monitoring plasma ( 1→3 ) - β - D - glucan is useful for the evaluation of therapeutic effect of antimicrobial medicine.
