摘要
目的:通过对儿童少年精神分裂症患者氯氮平治疗后血清6种常见酶的长期检测,了解用药对患者肝脏功能损害和适应过程。方法:选取未经治疗的首发青少年精神分裂症患者70例,于治疗前和治疗2周、3周、4周、5周、6周及7周以上时,检测患者血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)、乳酸脱氢酶(LDH)、γ-谷氨酰转肽酶(γ-GT)、胆碱酯酶(CHE),并同时于2周、4周、6周、8周采用简明精神病评定量表(BPRS)和副反应量表(TESS)评定疗效和副反应。结果:患者BPRS和TESS量表评分,治疗前后相比差异均具有显著性(P〈0.05),治疗2周与其他周相比差异具有显著性(P〈0.05)。ALT和γ-GT第2周显著升高(P〈0.05~0.01),第3周降到与治疗前差异无显著性(P〉0.05),第4周升高,与治疗前相比差异具有极显著性(P〈0.01),然后逐渐下降,与治疗前相比差异无显著性(P〉0.05);LDH第2周至第5周显著高于治疗前(P〈0.01),第6周后逐渐下降至与治疗前差异无显著性(P〉0.05);AST、AIP虽有先增高后恢复至略高于治疗前的趋势,但经比较发现差异无显著性(P〉0.05);CHE治疗前后始终无显著性改变(P〉0.05)。治疗第3周与其他周比较,ALT差异均具有显著性(P〈0.01);而其他酶的差异无显著性(P〉0.05)。结论:氯氮平治疗青少年精神分裂症疗效确切,引起的肝脏损害具有规律性.且较轻微,随着治疗的深入,人体可以逐渐适应用药状况。
Objective To test six familiar enzymes of hobbledehoy schizoprenics who were treated by clozapine dynamlyical,according the change of these indexes to find out the damage and the adaptability of the liver function. Methods Choose 70 hobbledehoys who were the first time suffer schizophrenia and had never been treated, measure their enzyme levels before and after 2,3,4,5,6 and 7 weeks of treated, the enzyme including ALT,AST,ALP,LDH,γ-GT,CHE,TBA,T-Bil, D-Bil, I-Bil, TP,ALB,GLO,P-ALB,and then measured the curative effect and side reaction using BPRS and TESS after 2,4,6,8weeks of treated. Results The BPRS and TESS of sufferer had significant difference between before treatment and after treatment( P 〈 0. 05 ). It had significant difference between the first 2 weeks treatment and the other weeks( P 〈 0. 05 ). ALT and γ-GT risen up significantly during the second week( P 〈0.05 -P 〈 0. 01 ). The significant difference dropped to the same as before treatment in the third week, during the fourth week, it rose up primarily, and had higher significant difference than before treatment ( P 〈 0. 01 ), and then drop gradually to the same as before treatment afterwards ( P 〉 0. 05 ). during second week to fifth week, the significant difference of LDH is higher than before treatment( P 〈 0. 01 ), it drop gradually to the same as before treatment after the sixth week( P 〉 0. 05 ). Although the AST and ALP had the trends of heighten first and then drop to a little higher than before treatment,it had no significant difference( P 〉 0. 05 ). The CHE always had no significant difference compared to it of before treatment(P 〉0.05 ). Compare the third week to the other weeks, ALT has significant difference( P 〈 0. 01 ), but the other items has no significant difference ( P 〉 0. 05 ). Conclusion It is certain that clozapine has an effect on hobbledehoy schizophrenics,The liver damage it caused had regularity, and was light, God's image can be adapt to the situation gradually with treat deeply.
