慢性心房颤动心肌中钙蛋白酶含量与心肌细胞重构及凋亡的关系 被引量:1

Relationship between calpains with remodeling and apoptosis in myocytes in patients with atrial fibrillation

摘要 目的探讨风湿性二尖瓣病变伴心房颤动患者右心房心肌细胞中钙蛋白酶的含量与超微结构的变化及凋亡的关系。方法风湿性二尖瓣病变行二尖瓣置换术患者30例,窦性心律(对照组)15例,合并房颤(观察组)15例。术中取右心房组织,用Western blot测定心肌细胞内钙蛋白酶I、钙蛋白酶Ⅱ的含量;透射电镜观察心房肌细胞超微结构改变;TUNEL测心房肌细胞凋亡指数(AI)。结果观察组钙蛋白酶I含量、肌小节溶解率(SSR)、凋亡水平均明显高于对照组(P<0.01);钙蛋白酶Ⅱ含量无变化;观察组细胞超微结构改变较对照组严重。观察组钙蛋白酶I含量与SSR及AI呈明显正相关(P<0.01)。结论风湿性二尖瓣病变伴房颤时右心房心肌细胞中钙蛋白酶Ⅰ含量增加,超微结构改变明显,凋亡水平上升,三者之间关系密切;钙蛋白酶I可能在房颤的发展及维持中起重要作用。 Objective To investigate the relationship of the content of ealpains, remodeling and apoptosis in right atrial myocytes in patients with rheumatic atrial fibrillation(AF). Methods Thirty patients with rheumatic mitral valve desease undergoing mitral valve replacement were divided into sinus rhythm group (group C, n= 15) and group AF(n= 15). Right atrial appendages were taken before cardiopulmonary bypass during operation. The contents of calpain-Ⅰ and calpain-Ⅱ were assayed by Western blot. Ultramicrostructure changes were observed by electron microscopy. The apoptosis index (AI) was measured by TUNEL. Results The content of calpain-Ⅰ,SSR, AI in group AF was significantly higher than that in group C(P〈0. 01). The content of calpain-Ⅱ was not remarkably different between the two groups. Ultramicrostructure changes were more serious in group AF than those in group C. In group AF, the content of calpain-Ⅰ was positively correlated with SSR and AI (P〈0. 01). Conclusion In the right atrium myocytes in patients with rheumatic AF, there is an increase in the content of calpain Ⅰ with ultramicrostructure change and apoptosis. Calpain-Ⅰ has a close relationship with ultramicrostructure changes and apoptosis, and may play an important role in rheumatic AF.
